IMPORTANT LYME DISEASE RESEARCH PROJECT...PLEASE PARTICIPATE

A project is being conducted to bring the needs of those with Lyme

disease to the attention of speech-language pathologists. For this

project to be successful, a large number of individuals with Lyme

disease are needed to participate. All participation involves is taking

10-15 minutes of your time to complete an easy online survey. If you

are over 18 years-of-age, please consider participating and accessing

the survey by clicking on any of the links provided in this message.

(Please be assured the information provided will be kept confidential by

the researchers and can in no way be used to individually identify you.

Further, completing this survey will not generate unwanted email

solicitations from the researchers or any third-parties.)

Survey completion is fairly quick and easy, though individuals should be

aware of the following instructions:

1. Once you begin taking the survey it must be completed in a single

sitting, as there is no provision for saving responses and returning

to finish at a later time. Though the survey should take only 10-15

minutes to complete, there is no time limit for completion and

respondent should feel free to take as long as they need to provide

well-considered answers.

2. To navigate through the survey click the " Next " button at the

bottom of each page. Only click this button whent you have completed

the page, as there is no provision for going back to a page that has

been finished.

3. When you have completed the survey and arrived at the final page

click the " Done " button at the bottom and close the window the survey

opened in.

4. To optimize viewing, please maximize window size if the survey does

not open as a full screen. Because of minor incompatibility problems

with some internet browsers the survy format and spacing may be

disrupted on your computer. Please be assured that this should not

affect you ability to take the survey or compromise your answers.
