Suggestion for all that I personally found helpful with medical records / accuracy: At each medical appointment I give the MD a 1-page 'summary' of what I want to go through during the appointment and have them numbered. This includes: my current medications; supplements; next scheduled appointments; recent lab work I need results for (while my sheet shows the past/previous results); symptoms; and anything else relative to my appointment or care (i.e. Lyme medical abtracts and citations). She spends the first 2-3 minutes reading through this and then starts to answer / address the items raised... and I have a copy to write my notes / her reply on. We have both found that the appointment is not stressed and we get more accomplished. By her having this 'summary' in my medical file it makes it easier for her to fill out any medical realted forms; compare labs; and have a handle on my situation (i.e. not confuse me with another patient).
