I have found that starting low- very low- and creeping up on the desired dose works well. Of course, if you have bad symptoms, impatience is understandable, and I really can't advise other than to say try 1.5 or 3.0 mg first. The second recommendation would be to cut out starch and sugar. Eat a low-carb diet. If in doubt, read " Good Calories, Bad Calories " first. I follow only two rules: grain-free (no wheat, no rice, no barley, no corn, no oats, etc.), and low-carb (under 150 grams/day). If one were really hurting, I'd say under 72 grams/day, as per Lutz (Life Without Bread- a highly recommended read). Good luck and best wishes.
