Do not mention your doctor's names, this is a time to protect them, not expose them. 2. Be polite and refrain from using inflammatory remarks. Thank them for their time. 3. If you had trouble being diagnosed, or finding a doctor to treat you past 28 days state it, as these are critical problems many have faced. 4. If you have had trouble with getting your insurance company to pay for medicine or tests prescribed by your doctor, please state it. 5. State clearly (if you have been) treated with long term antibiotics. Point out the effects of this treatment, such as; significantly improved, cured, able to return to work, a 50% improvement, etc. 6. Add in something personal. How did LD change your life style, finances, home life, and even your hobbies. Did you pass LD on to your child. 7. Alert them to the fact that doctors are being investigated for using the same protocol that improved or cured your LD. 8. Include your name and address.
