I sit in my chair sometimes and just stare in space for hours. Then I wonder where the day went. My daughter doesn't understand why I can't get up and do dishes or take out the trash or cook. In my case, my 'remission' is based solely on stable LFT's, and not on the level of inflammation or shape that my liver is in. But find it very difficult to not beat myself up daily over guilt that I actually try to justify for being lazy, or tired, or just not feeling right. I never had positive ANA or Smooth Muscle tests until just recently (despite 'remission'). It was one reason why it took a year to get a positive diagnosis. In my case they used process of elimination. My diagnosis was confirmed by Dr. Carithers (a renowned hepatologist in Seattle). It surprises me that your doctors would doubt your diagnosis just because of those factors, especially when they are not present in all people with AIH. I spent the 1st 2 years beating myself up because I was too " lazy " to do anything. With the help of this and other groups, I now realize that the fatigue is real, and extremely debilitating. Until you have experienced the kind of fatigue that we have, no one can know the effort we make every day to just do the things we CAN do. We are far from lazy. Guest guest Posted November 17, 1999 Report Share Posted November 17, 1999 Kathy, It seems to me that I had anti-smooth muscle whatever and positive ANA (haven't a clue what either of them mean though Don has tried to get it through my muddy skull previously). But these are things that they only tested me for when I was first sick, so they must be something that once they're there, they're permanent? The impression I've had is that AIH diagnosis is based on process of elimination more than anything else. If you have elevated or lowered LFTs, inflammation, cirrhosis or fibrosis, perhaps fatty liver, but do not have antibodies that indicate that you've ever had Hepatitis, they start looking for an autoimmune condition. Somewhere in my vast and nearly useless store of AIH information I read something about how certain LFTs are extremely high for PBC or PSC where they run lower (but still elevated) for AIH. Not all of the elevations are viewed with alarm, which surprises me. Also, there are several different types of AIH but I've read little about that and would like to know more. He explained to me that it will take a whole year of remission for the inflammation to go down. Also, the first the he said to me was I want you to have Hepatitis A and Hepatitis B vaccinations so we don't add insult to injury. Also, wants me to get a flu shot and pnemonia shot just to be on the safe side. I went ahead and got the Hepatitis A shot Tuesday and it had no side effects, just a really sore arm since they do it in your muscle. I will get the Hepatis B shot next month. I always have elevated GGTP and my glucose and triglycerides seem to swing wildly, but only on non-fasting blood tests. Otherwise, almost everything stays in normal ranges. Remission, as I understand it, isn't assumed until they've been in normal ranges for a prolonged period. Six months? Not sure. Still, if they have been steadily declining into normal ranges, that seems like it's a good sign. Is that what's happening for you? I hope so! Our neighbor's cat hangs out and waits for the opportunity to drop in and socialize with us, maybe sample our cat's food, tease our cats a little. He's really a sweet cat and we'd adopt him if he needed a home, but in more than a year of him hanging out here the cats have never become friends. I think that most of us with AIH do, but it's my understanding that " remission " means that the disease is no longer progressing, just kind of stopped in it's tracks. I haven't had normal LFTs in two years yet I'm in remission and my liver was pretty well shot when I was diagnosed - stage IV cirrhosis and the scale only goes from I to IV. Even with slight regeneration I don't think total regeneration happens without a trip to Lourdes!! Most of my LFTs are normal, by the way, but my GGTP is always elevated (that indicates liver damage) and my Alk. P is usually elevated. Glucose and triglycerides are generally elevated, too, since a damaged liver can't process glucose and cholesterol very well. I'm happy to accept remission, such as it is, instead of anticipating a transplant, though who knows what a new transplant center will say? So many questions and variables we all have to deal with! My wife and I lived for two and a half years in a home with 4+ stachybotrys, asperllius, penicilium, chartium and tachederma.
