This test must be paid in advance via cash, check, or credit card at the clinic. ---Clinic Costs--- Dr. Bergeron & Terry 's initial 60-minute consultation is charged at $250 and is due at the time of the Outreach Clinic appointment via check, cash, or credit card. The phlebotomy fee is $30. An experienced pediatric phlebotomist will perform all necessary laboratory procedures for blood & hair collection. The HELPcard: If approved, the HELPcard can be used as a credit card for healthcare financing. Call the Outreach Team for details at 859-261-4503. For testing fees: See discounted test panels. All other specimens (urine and stool) will be collected at home. Any other recommended test kits will be available at the clinic to take home for collection. Prepaid FedEx envelopes are in all kits to return specimen(s) to GPL. Test results will be sent to Dr. Bergeron's office within 4-6 weeks. The follow-up appointment with Dr. Bergeron & Terry is billed separately at an hourly rate of $250. The follow-up appointment will take place in Dr. Bergeron & Terry 's private office to review laboratory test results. After reviewing test results, Dr. Bergeron & Terry will develop a treatment plan for each patient individually. ----Insurance Information--- BlueCross BlueShield PPO is the ONLY insurance accepted at outreach clinics for laboratory testing and phlebotomy fees. To determine the amount of coverage, contact BCBS PPO and provide them with the CPT Codes, which will be provided in the confirmation e-mail once the appointment has been scheduled. Most testing is covered; however, it is not guaranteed that BCBS PPO will cover any or all testing at 100%. It is the patient/parents' responsibility to check with BCBS to inquire about covered costs & co-pays/deductibles. Please be advised: We are unable to accept Medicare, Medicaid, Champus or Tricare plans for payment for any fees or services at the Outreach Clinic. Special discounted prices do NOT apply to anyone for whom insurance is filed. If insurance is NOT being filed, the total amount is due at the time of the appointment and may be paid via check or credit card. A medical receipt with CPT and diagnostic codes will be provided and may be submitted to your insurance company (by you) for reimbursement. Cancellation Policy: A minimum of 15 patients must be signed up 14 calendar days before the clinic date. Such policies must be enforced in order to prevent last-minute sign-ups and to ensure accurate planning. This does not mean that appointments will not be taken after the 14-day cutoff; however, it is asked that patients schedule appointments as soon as possible in order to ensure the clinic will take place. In the event of cancellation by the patient, please contact The Outreach Team at 859-261-4503 at least 72 hours prior to the appointment.
