Outreach Clinics offer families a full service opportunity to begin biomedical

interventions by providing an on-site physician, an experienced pediatric

phlebotomist, and comprehensive biological testing panels at discounted prices.

In accordance with the comprehensive approach, a special panel of tests has been

specifically designed to detect the most common imbalances associated with

nutritional deficiencies, immune issues, digestive disorders, metabolic

imbalances, allergies, infectious conditions, and issues associated with

oxidative stress. Performing multiple tests increases the chances of identifying

all areas likely to contribute to symptoms at hand and knowing the full scope of

the patient's metabolic state will assist the physician to better design a

treatment plan that is specific to the patient's needs. Restoring balance to all

functions of the body produces a synergy that cannot be achiev ed by addressing

abnormalities individually. The physician will address any concerns about the

relevance of specific tests.

Rhett Bergeron, M.D., graduated from LSU Medical School, at which time he

realized inadequacies of conventional medicine based on drug therapies alone and

the realistic possibilities of healing the body with nutritional and

metabolic/immune therapies. He began studying metabolic and immunological

medicine and applying nutritional therapies to patients with success; however,

after recognizing the complexities of many more advanced disease conditions, Dr.

Bergeron joined an immunology research lab to studying the immune system in

chronic fatigue, fibromyalgia and cancer. In his current practice he integrates

nutritional, metabolic, biological and immune therapies with conventional

medicine in the treat ment of chronic diseases, with an emphasis is on

identification of underlying causes of disease, and patient education and

responsibility.

Terry , B.S., an enthusiastic Nutritionist with over 17 years experience

helping people to better health, including healing his own son Noah of Asthma.

Some modalities that he employs include hair-tissue-mineral analysis, iridology,

detoxification, Bio-terrain testing, salvia hormone testing, food allergy

testing, oat testing, and raw food therapy. Terry has a private practice and is

a certified massage therapist and yoga instructor. He has successfully worked

with many conditions including allergies, asthma, ADD and ADHD, autism, thyroid

problems (hypo and hyper) chronic fatigue, fibromyalgia, heavy metal poisoning,

lupus, depression, hormonal imbalance, cancer, ar thritis, skin disorders, and

cardiac problems.

At the clinic, there are two panels being offered: the Comprehensive Autism

Panel and the Basic Autism Panel. Other panels and individual tests are also

available. Please note that the list price means insurance price.

The Comprehensive Autism Panel is offered at a patient discounted panel price

of $1,453 for a savings of $363, which only applies to the patient cash price

(credit card or check).

* Please note: The IgG Food Allergy Test may not be covered by Blue Cross/Blue

Shield PPO insurance. This test must be paid in advance via cash, check, or

credit card at the clinic.

---Clinic Costs---

Dr. Bergeron & Terry 's initial 60-minute consultation is charged at $250

and is due at the time of the Outreach Clinic appointment via check, cash, or

credit card.

The phlebotomy fee is $30. An experienced pediatric phlebotomist will perform

all necessary laboratory procedures for blood & hair collection.

The HELPcard: If approved, the HELPcard can be used as a credit card for

healthcare financing. Call the Outreach Team for details at 859-261-4503.

For testing fees: See discounted test panels.

All other specimens (urine and stool) will be collected at home. Any other

recommended test kits will be available at the clinic to take home for

collection.

Prepaid FedEx envelopes are in all kits to return specimen(s) to GPL.

Test results will be sent to Dr. Bergeron's office within 4-6 weeks.

The follow-up appointment with Dr. Bergeron & Terry is billed separately

at an hourly rate of $250. The follow-up appointment will take place in Dr.

Bergeron & Terry 's private office to review laboratory test results.

After reviewing test results, Dr. Bergeron & Terry will develop a

treatment plan for each patient individually.

----Insurance Information---

BlueCross BlueShield PPO is the ONLY insurance accepted at outreach clinics for

laboratory testing and phlebotomy fees.

To determine the amount of coverage, contact BCBS PPO and provide them with the

CPT Codes, which will be provided in the confirmation e-mail once the

appointment has been scheduled. Most testing is covered; however, it is not

guaranteed that BCBS PPO will cover any or all testing at 100%. It is the

patient/parents' responsibility to check with BCBS to inquire about covered

costs & co-pays/deductibles.

Please be advised: We are unable to accept Medicare, Medicaid, Champus or

Tricare plans for payment for any fees or services at the Outreach Clinic.

Special discounted prices do NOT apply to anyone for whom insurance is filed.

If insurance is NOT being filed, the total amount is due at the time of the

appointment and may be paid via check or credit card.

A medical receipt with CPT and diagnostic codes will be provided and may be

submitted to your insurance company (by you) for reimbursement.

Cancellation Policy: A minimum of 15 patients must be signed up 14 calendar days

before the clinic date. Such policies must be enforced in order to prevent

last-minute sign-ups and to ensure accurate planning. This does not mean that

appointments will not be taken after the 14-day cutoff; however, it is asked

that patients schedule appointments as soon as possible in order to ensure the

clinic will take place. In the event of cancellation by the patient, please

contact The Outreach Team at 859-261-4503 at least 72 hours prior to the

appointment. Failure to provide ample notice will result in a charge to your

credit card in the amount of the physician's fee.
