Dear Parents,

Thank you for your interest in University of Washington's Play Lab. The

Play Lab will be staffed by Dr. Myers, Ph.D. from UMW and DeCou,

M.Ed., LPC from Exceptional Support Services in Fredericksburg. The Play Lab

will be held at Helping Hands Pediatric Occupational Therapy in Fredericksburg.

Each semester the Play Lab class is held, UMW students will be focusing on a

different theme and different research-based interventions, geared to meet a

variety of the special needs related to children with developmental

disabilities. This fall the theme is " Movement and Manners " and is geared

toward children ages 3-8. We will be working on sensory strategies, behavior

skills, academics, and social skills development as part of this Play Lab. We

are looking for children with diagnoses such as: Intellectual Disability, Autism

Spectrum Disorder, Developmental Delay, ADHD, and Sensory Processing Disorder.

We invite you to submit an application for your child to participate in this

fall's Play Lab. Applications are due Friday, September 10, 2010 at 5 p.m. The

number of children approved to participate in Play Lab each semester depends on

grant funding and UMW student enrollment. We anticipate a high level of

interest in this program however there will be a limited number of spaces

available and applications must be reviewed by a UMW faculty committee before

families are notified whether they can participate in a Play Lab session. If

your child does not participate in this Play Lab session, we encourage you to

apply for an upcoming Play Lab (fall and spring).

Please review the list below to determine if your child/family meets the

criteria for participation. Feel free to contact us with any questions at

Criteria:

• Ages 3-8 with a diagnosis of developmental disability (ID, ASD, SPD,

ADHD, etc.)

• Available for Play Lab on Mondays from October 18-November 15th,

6:00-7:15 p.m.

• Copy of I.E.P and application received by September 10, 2010 by 5 p.m.

• Parent available for Play Lab Parent Training on Monday, November 22,

2010 at 6 p.m.

• Parent must agree to allow for child to be videotaped/photographed for

instructional and marketing purposes.

• Once selected children are accepted and parents are notified of

acceptance, tuition payment of $150.00 written to UMW Autism Clinic and sent to

Dr. Myer's address below must be received by September 24, 2010 by 5p.m., to

participate and hold your spot in Play Lab. If payment is not received by the

deadline, another child will be given the slot and have the opportunity to

participate in Play Lab.
