His office is in PA (I'm in Indiana so I did a phone consultation). He charges $7.25/minute for the phone consultation. When you call them for the first time, they ask you if you have a fax number where you can receive faxes. They fax approx. 7 pages to you for you to fill out - medical history, medications, allergies, credit card number, etc. Then you fax it back to them. When they receive the fax, they call you and schedule a time for you to call and do the consultation (and boy, they were quick too - I called them on Friday and was scheduled for today, Tuesday!). > > So for those of you who don't have fax machines, I have a couple of recommendations. > > 1.
