Did you have in the past positive skin test (mantoux) Then you must visit pneumologist Doctor asap. He is the appropriate person for reading X-ray Tuberculosis has specific sympomts.Chest pain,hemoptysis, fever, chills, night sweats, appetite loss, weight loss, and easy fatigability. I haven't had a positive skin test ever but I have not had one since these symptoms of chest pain occured...... I do have chills at times, night sweats at times. I'm always tired but that's nothing new.... Maybe I'm a little slow, dense or uninformed, but I haven't a clue... and Spot is looking at me with the most curious look, kinda like " what the heck are you asking a stupid question like that for???? " I'm sure he figures he knows, so everyone else should... (I said he was lazy, not stupid... actually quite the opposite... Thor's the dense one...) anyone, can anyone give me an answer??? Guest guest Posted August 27, 2006 Report Share Posted August 27, 2006 I have had many bouts with pnuemonia due to the higher infection rate of Enbrel. Last year I had pnuemonia that my doctor did not think I would survive. At first they thought it was TB, a concern for all of us on Enbrel and other like drugs. Many people carry the TB virus(?), and is " stored " in a " little jail cell " in the lungs, even if we have never had TB. Enbrel breaks down the " jail cells " , and releases the TB into our bodies. Some people will not test positive for TB, but still may be a carrier. Thats the simple explaination that was given to me by my doctor. The doctors were very happy to discover I did not have TB, apparently its a major deal at the hospital. Everyone (and their families) that came close to me would have had to be treated, along with the hospital and the equipment. It's not so much that TB and pneumonia have anything to do with PA. But if you have chest symptoms that can indicate either pneumonia OR TB (lingering cough for several weeks, weight loss, night sweats, low grade fevers, coughing up blood in the sputum, etc.) it is important to determine which one you have. For pneumonia, it doesn't affect the choice of which medications you can use to treat PA, except that you cannot begin the biologics until after the infection is cleared with a round of antibiotic therapy. With latent TB, however, you must complete a course of anti-TB medication and have a negative chest X- ray before you can start taking ANY biologics. (I don't believe taking Methotrexate is affected by this, just the biologics like Enbrel, Orencia, Humira, and Remicade.) If you do have latent TB and aren't treated before beginning the biologics, they can suppress your immune system enough so that the TB can become active. Active TB primarily occurs in the lungs, but there have been cases where it has occurred in the bones. (That's what Saint Bernadette of Lourdes died from; TB.
