Most of what I've read is that for those who's hypogonadism is secondary (Hypothalamus/Pituitary in origin) fertility will return when TRT is discontinued. It's a lot like when a woman is taking hormones as birth control and then when they come off of them their fertility comes back. Sometimes it can take months and months to restore fertility but with the use of HCG and other hormones fertility can be stimulated more quickly. The bottom line is that if the hypogonadism is primary (meaning disfunction of the testicles rather than the hypothalamus or pituitary) then fertility can be a long term issue even after TRT is discontinued. The reason for this is that whatever was causing the testicles to produce less and less Testosterone would likely continue to progress regardless of whether someone was on TRT or not. So when coming off of TRT the testicles would even be less able to produce T even if the gonotropin hormones were restored. If your hypogonadism is secondary then there's likely nothing to worry about long term. If it was primary and you still may want to have children then I might suggest talking to your doctor about doing a fertility treatment to get a sperm sample frozen before going on long term TRT. Since it wasn't entirely clear what the cause of my hypogonadism was I had my sperm frozen just in case I don't get fertility back again. I was infertile on 10g / day of Androgel and have continued to be infertile on IM injections. I don't know if my fertility will come back someday if I want children but I'm glad I have some sperm frozen just in case.
