More than likely you will need to take HCG also to get your testes back up and running to produce what T/sperm you were prior to taking the Androgel. Unfortunately, yes your testes will shrink/atrophy due to the exogenous T you are taking. Once the Hypothalamus sees the external T enter your blood stream it'll think it's from your testes and thus quit sending the signals to make more as it senses there's enough T in the system. It's quite elemenatary when you think about it...but scary as this is our body/mind we are talking about! Many posts on here talking about doing HCG along with their TRT protocal. It's something to look into. This prevents the testes from shrinking as the HCG mimics the signals given by the Hypothalamus and thus your testes continue to work as they were (how much depends on your dosage of HCG). Also, if you are secondary, you may be able to suffice on HCG alone. What are your test results prior to TRT? LH/FSH/Prolactin/E2/Total T are the key ones. So far, the only fix is to use HCG (human chorionic gonadotrophin) which mimics LH and this tells your testicles to start producing T so the enlarge as part of that process. Some guys have more shrinkage than others and who knows why. I have been on TRT for almost 2 years and mine are about half the size of what they were prior to TRT. Normal - for whatever that is worth is suppose to be about like a medium sized pecan in the shell for an example. Most of us on TRT have ones the size of a medium grape or smaller. Experience such as Phil has shown that even though they atrophy for several years, use of HCG can bring them back. Some are even considering cycling the HCG (use for a month, off for a month). A lot of Dr.'s just test Total T and put men on TRT what you need to know is why your low. A lot of things can do this Thyroid, Low or High Cortisol levels, Pituitary Problems, Fatty Liver, Stress, Drugs the list is dam long. Most young men can have kids on the right TRT with there levels right. But I would have some sperm tested before gong on Andorgel to be sure they are good if so have some frozen just to be safe. We all are not the same some men don't get small testis on TRT most do adding HCG will keep them working. If you ask you DR. about this he will think High dose's like 2500 IU's this is not what we do. On Andgel you would do a shot every 3 day of a low dose 250 IU's. Also one needs to be tested after starting Androgel in 2 weeks to be sure it's working and it's the right dose. You can go up then drop back down lower then you base line test. Your body sees the added T and stops sending the messages to your testis to make more T so you drop back down some go back down lower because the dose they need needs to be higher. Also some the gel does not get through the skin. Dr.'s that put new men on this and say come back in 3 months do not know what they are doing and only know what the Drug Reps tell them. I was wondering, if your testes were born smaller or abnormal, would HCG still be an option? Given, Im running out of reasons to give to Dr concerning trying HCG, maybe the atrophy would work. Dan Hey Dan, I have been on T for over 20 years don't know how much longer that is as far back as my Dr's records go. I have have lost quite a bit in size of my testicles. They are about the size of almonds now. Since I am 83 and really don't intend on having any more babies, Ha! Is there any reason that I should take HCG to increase the activity of my testes? Would I feel any different? Also when I had my complete bloof work done in March 2006 my FSH was low .37 range 1-6 My LH was also low 1.54 range 2-12. Is this because my testes are not putting out? my Prolactin was in midrange at 9.08 range 1.81-18.77 I am not sure what Prolactin does but assume it is a hormone too. Should it be they be in the upper 1/3 range like Total T? If I don't need to take HCG I sure won't as I am taking enough mecications already. I hate taking all this junk but it seems necessary to feel well. I am not having any morning wood lately so I have gone back on Arimidex. I have extended the Arimidex to evey 10 days now that I went down to 6 when I took .25mg evey week so I am still tryng to get it balanced out. I see all you older posters are still trying to get yours right too. I would guess from that it is a continually balancing project, right? Just some questions in my mind. I see you and phil are still trying to get to the proper balance although Phil is haveing pitutary problems that he didn't know about, quite different than yours. In the first case (vanity), none of us want itty bitty balls. After all, is this not a sign of manhood? Well I guess its all in your perspective. Right now I am more concerned with being able to make love to my wife so that we both get enjoyment and satisfaction. I will admit that I would like to have bigger ones - don't we all. If this is the case, you have 2 options, take HCG which will enlarge them if they are still capable or get them removed and replaced with artificle ones similar to breast augmentation. They install smaller versions into your scrotum. I am not in favor of this unless it is warranted due to cancer or some other health risk. Option 2 is the philosophy that if you don't use a part of your body, your body will slowly get rid of it. This is like when you break a leg and put a cast on. When you take the cast off you look down and go where did my leg muscles go? Same principle. I really think that if you can keep your testicles operating on some level it is better for you than just not using them. I have been really thinking hard about doing this. Like you, I hate to have to take another drug but it probably is beneficial for you. We really don't know what all LH does for you besides making your testes produce T. Most say they just feel better when taking it. Once again it's a complicated balance because you will increase your total T which means more E2 and DHT conversion issues unless you drop your shots by some amount equal to what you would naturally produce. The levels of the different hormones vary - they should not all be in the upper third. Prolactin is a hormone but it really isn't understood what it does for men. In women it starts the milk production process (lactation). How expensive is HCG to buy? I would have spend out of pocket,this is my main concern The cost verses the results. Guest guest Posted September 7, 2006 Report Share Posted September 7, 2006 Elliot you need to understand it's not just about your testis when we go on TRT, are HPTA gets suppressed and we also suffer decreased Pregeneolone levels, which is the first step after CHOL in all three hormoal pathways which begin with CHOL, HCG increases Pregeneolone production, and therefore restores a more natural balance to our hormones. So adding HCG gives you an increased sense of well-being and also libido. Also we instinctively do not want all those LH receptors (including those we have yet to discover and appreciate) unstimulated. Guess you still don't know how much HCG cost as the VA won't supply it or Arimidex or Cialis, only testosterone. Guess that is the only thing they think we need treated for like all Va programs. I hope someone on this board knows how much it does cost and will post a price. Wouldn't be bad if you could alternate it every other month as your Doctor has suggested, and found it is working for him. I guess you need a script for it too of course. By the way I have a deeply ingrained virus in my computer, man is going to take it to work with him and I won't be on for 2 maybe 3 days, so you won't hear from me. He is picking] it up Tuesday. I am not on TRT, but my scrotum has shrunk a lot. However, my testes are still in the normal size range. However, they have lost their firmness. The testicles didn't shrink much, if any in size (thank goodness) they were just nestled close to the pelvic bone, no matter what the outside temperature was. This left the scrotum as just a smallish dense lump; the whole scrotum sac was reduced to the size probably equal to one testicle alone, by comparison. Does any of this sound familiar. I know the big difference here is that you're not on TRT, so how much this information is worth is in doubt. When I started taking HCG along with the TRT, the testicles started returning into the scrotum sac. The first week or so was a bit uncomfortable when this was going on; the whole area around the groin was sensitive. Let's say I wasn't crossing my legs a whole lot. The scrotum and testes now act normal these days; extended when its warm out, and retracted when its cold. The sperm cound drops and you can become sterile maybe in as short a time as a year or two. I do not have an specific information on that. I have been on TRT since 1991 and one testicle was damaged while in high school so it was already about the size of a grape. The good one lasted until we had 4 kids and I reached 60 before it stopped working as it should. As I didn't have any choice in improving the performance it did not seem a loss when I found I felt so much better with my level of T now above 800. The good testicle shrank from the size of a golf ball to about the size of a grape. There are a lot of other cell in the body that are stimulated by LH then the Testis. You and your Dr. need to catch up on the new things going on with TRT. Dr.'s are finding out that shuting down LH has a bad effect on other glainds in the body Thyroid and Adrenals for a starte. The good one lasted until we had 4 kids and I reached 60 before it stopped working as it should. As I didn't have any choice in improving the performance it did not seem a loss when I found I felt so much better with my level of T now above 800. The good testicle shrank from the size of a golf ball to about the size of a grape. This mess with are Thyroid, Adrenals and God knows what gets messed up. So doing HCG with TRT keeps at least the LH cells working along with the testis. Are Endo's allowed to provide hCG therapy now? Some doctors prescribe an approved drug when is it not approved for this application yet? Are there studies going on that show LH and FSH is enhanced? HCG only effects the Pit. to produce LH? After reading " The Testosterone Syndrome " I was surprised to find out how essential T is to many vital organs. It is very technical so let me ask if I understand right. 29 men were given both 200 mg of T every week and hCG in sufficient quantities to maintain the sperm developement in spite of the normal effects of shutting down the testicles. Am I correct that hCG acts as alternative hormone to either LH or FSH? Is the pituitary shut down by TRT or just the messanger hormones for the testicles?
