Guest guest Posted May 17, 1999 Report Share Posted May 17, 1999 Elena, I have found that if I take a large dose of ibuprofen or Vicodin I can feel it in my liver area. I was having bad uterine pain (due to fibroid tumors) one day so took Vicodin, that pain went away but then my liver hurt for the next 24 hours. I don't know which was worse. I usually don't have liver pain anymore since starting on the prednisone, but I guess my liver is still very sensitive to drugs. I don't drink alcohol but I imagine that it has the same effect as any other drug since it must be cleansed from the body by the liver. Jo from N. Hes answer was that because i am aware that I have AIH i will feel pain. In other words im imagining it. I am having fatigue also, i mentioned this to another doctor and he told me to exercise more!! On a different subject, I wanted to ask everyone if they have experienced one of my symptoms. For example, the other day i had a fruit smoothie for lunch, 2 hours later i started getting very shaky, my hands trembled and I was kind of lightheaded. I got something to eat and my symptoms went immediately. I know this wasnt because i was not eating enough because i had a good breakfast etc. I have experienced that episode twice now. Secondly, I remember someone on the list mentioning that they had saw Dr Ritt in san diego; I will be seeing him soon and I wanted to know what hes like. My doctor attributes it to low glucose (blood sugar) which usually happens when my hepatitis is as he calls it " flaring up " . I noticed i usually get the shakiness in the middle of the night. But at the time my glucose levels dropped to in the 50's range my LFT's were doubled from two weeks prior. He increased my prednisone from 10 to 20 mg. I am presently on 15 mg prednisone (I decreased the doseage myself ... only I know my own body right?) and also on 50 mg Imuran and 40 mg of Lasix. I am level IV cirhosis with approximately 12% functioning liver capacity. One prior major esophageal bleed in 1994 (which nearly killed me). I dont know if it was the increase in the prednisone which caused my LFTs to stabilize or if the " flaring up " period passed on its own. I really dont pay much attention any more. LOL Having had this disease since 1985 (at least diagnosed then .. but i'm sure I had it since at least 1980). I am approaching my 38th birthday (next Sunday .. ohhhhh boy LOL). One other thing ... my doctor also advised me a long time ago that you will not feel pain in your liver .. but from the surrounding tissue. I too get the pains on the right side just under the rib cage as well as on the left side near the spleen. Although at times I've been told it's psychosematic ... we know better right? I agree with whoever posted the comment .. let the doctors feel what we feel for one day .. they'll finally realize what it's like. Guest guest Posted May 19, 1999 Report Share Posted May 19, 1999 Debbie, Your condition is apparently similar to mine since I'm also in stage IV cirrhosis. I've had ongoing problems with pain on my left side but only rarely and very little on the right. My doctor(s) haven't dismissed my complaints of pain at all. In fact, I'm lucky that they no longer dismiss much, if anything, as psychosomatic. I guess it takes time for them to decide whether they want to label us as just another " hysterical woman " . Or, maybe it has something to do with their relationships with the women in their lives - assuming the doctor is male. Because the pain on my left side has been frequently severe, my doctor ordered a colonoscopy which I declined (after much discussion.) He ordered an ultrasound at my request and my spleen was not enlarged. He said that the only organs on the left side where I was feeling pain are the spleen and the left liver lobe. However, in advanced cirrhosis, the liver shrinks and he said there is scarcely any liver there - not enough to cause the pain. Obviously, he does think there can be pain from the liver. Also, from the beginning, every doctor I've seen has poked and prodded in my liver area, asking me if it hurts. So, we can surmise that they think it's possible to feel liver pain. Six years ago my Mom almost died from a badly infected gall bladder and her doctors claimed there was nothing wrong with her until the night they finally got the picture that she was dying and did exploratory surgery. Medicine is not an exact science and the best doctors seem to realize this. The rest of them seem to go by the book, and that's not good news for those of us who put our lives in their hands. My Doctor believes I have pain under my right breast?? and he takes it very serious, it tells him that it's tender and I'm sick!! lol don't know!! My doctor would never tell me that it was in my head. His explanation for most weird pains I get, is this is a strange illness and we are on strange meds for it, so we should expect some strange things to happen?? If I have pain he believes me and looks into it! He admits he knows less about this illness then I do now, and now my gp, internist and my self work together to find the answers, that's all we can do!! At list my Doc listens?? I have also had the bumps around the face once and a butterfly rash when I got out of the shower that burned like a sun burn and was swollen (went away after an hour or so). The itching keeps me up at night and suffer from chronic fatigue - like I have a 300 pound weight on my body. Makes it hard to move - makes it hard to WANT to move. I also get migraine headaches. I used to think that, when I felt really bad, that I was coming down with the flu or something but now that I have heard what others with AIH feel like, I realize that it is simply the effects of AIH. The problem is that, one of these days I'm going to have something else and not even know it. Hang in there. I was in bed for ever it seemed like, because it hurt to move. since I have gotten treatment I haven't felt near as bad. It can be a major pain in the ***. The weird thing is, that I can sleep all day, I don't really I just feel like it. And as soon as bed time comes i am wide awake. BUT I grew up in Oregon (Eugene) and my parents and two sisters (and their families) have been living in Bend. I went to the University of Oregon in Corvallis, and to nursing school in Bend. I'm not sure what RUQ is, but I have pains sometimes when I try to take a deep breath, too. Also weekness and at times just feeling blah - it can come on rather suddenly. HEPATITIS/LIVER PAIN? Many chronic hepatitis C patients have expressed their frustrationwhen it comes to describing and sharing the experience of various abdominalpains and discomforts with their physicians. Frequently, these acute orchronic pains are dismissed as having little or nothing to do with chronicliver disease. What do physicians know about abdominal pain? Read thefollowing and learn about the mechanisms of abdominal pain in general and thetypes of pain that result from diseases of some specific organs. Abdominal organs are unresponsive to many stimuli that normally would elicit severe pain. For example, cutting or crushing of abdominalorgans does not result in a recognizable sensation. The pain fibers in theviscera (large interior organs) are generally sensitive only to stretching or increased wall tension. The causes of stretching or tension vary from intense muscular contractions. In hollow organs such as the intestine or gallbladder, nociceptivefibers are located in the muscular wall. Afferent (conveying towards acenter) impulses travel along the sympathetic nerves. In solid organs such asthe liver and kidney, such fibers supply the capsule and react tocapsular stretching. An inflammatory reaction to microbes or toxins producesorgan pain (visceral) through the elaboration of tissue hormones ormetabolites (substances produced by metabolism). Four different mechanisms giverise to abdominal pain: (1) visceral; (2) referred; (3) parietal peritoneal;and (4) psychogenic. Visceral Pain Pain resulting from stimulation of sensory afferent nervesinnervating abdominal organs. The pain is often difficult to describe (usually as cramping or aching), dull in nature, and poorly localized to themidline from the upper (epigastrium) to the lower abdominal area. The painmay be accompanied by nausea, vomiting, sweating, pallor, and restlessness. Patients often move about in bed, occasionally finding relief with achange in position. Referred Pain Visceral pain may be referred to a remote area of the body, where itis perceived as cutaneous pain (sensation of pain in the skin) in anarea supplied by the same spinal cord level as the affected abdominalorgan. Referred pain is usually well localized and appears when noxiousvisceral stimuli become more intense. Thus, swelling of the liver capsule by a hematoma (swollen blood vessels) after liver biopsy is firstperceived in the abdomen but may be referred to the right shoulder. Parietal Peritoneal Pain When the parietal peritoneum (abdominal membrane that encloses thatbody cavity) becomes involved as a result of abdominal pathology (disease process), nerves supplying the area are stimulated and generallyproduce pain that is more intense and more precisely localized than isvisceral pain. The classic example is the localized pain of acuteappendicitis. Parietal pain is often aggravated by movement; hence the patient'sdesire to lie completely still. Psychogenic Pain This is obviously abdominal pain that is perceived but without anylocal cause. Unfortunately, this may be a pain mechanism that somephysicians choose to attribute to some chronic hepatitis patients' episodes ofpain. However, as cited above, physicians should take the time to explainand concede that there are valid causes for different types andintensities of abdominal pain that arise from our internal organs due toinflammation and toxic conditions. Furthermore..... When the hollow structures of the gallbladder and biliary tractdilate due to the disease process, pain is experienced in the upper abdomen orright upper abdomen. Pain may also be referred to the back between theshoulder blades. Pain from the pancreas is also felt in the upper abdomen andis often referred to the middle of the back. In a manner analagous tothe liver, gallbladder, and biliary tract on the right, lesions in thetail of the pancreas that involve the diaphragm, may result in referred painto the left shoulder. Bacterial or viral infection of any intraabdominalorgan may cause abdominal pain. Interference with venous or arterial blood flowcan affect the abdominal organs. Clinically this may present as severe abdominal pain and shock. Drink at least eight to twelve glasses of filtered water daily.... Avoid eating large amounts of sugar.... Don't become obsessed with measuring calories.... Avoid foods that you may be allergic to.... Be aware of good intestinal hygiene.... Do not eat if you feel stressed or anxious.... Check if organically grown fresh produce is free of pesticides and available in your area. Obtain your protein from diverse sources (including legumes), not just from animal products such as meat, eggs, and fish.... Choose your breads and spreads wisely.... Avoid constipation by eating plenty of raw fruits and vegetables.... It has been kind of bad lately, and I don't know why. Guest guest Posted January 12, 2000 Report Share Posted January 12, 2000 I don't know if anyone on this list has liver pain on a regular basis. I've had it on and off for three years. Blood tests normal, liver scan normal. I've been taking milk thistle lately and it really does help keep the pain away. Of course if you have new liver pain, get it checked out with your doc, but if it's been around and your doc isn't offering any suggestions for relief, you might want to look into milk thistle. Only, I would get a stitch in my side after a couple of runs and had trouble being able to run. Later had trouble with same when I tried to learn to fast dance as 12 or 13. Had my gall bladder tested by sonagram then special sonagram to take a look at everything in that area, and then the gall bladder chemically stressed for about 10 minutes. Not fun, but I did find out my gall bladder was the source of the terrible chest wall and rib pain I'd suffered so very many years. I want to say the second test was called Heeat, or something similar. I'd eaten steamed foods since early 20's and knew I had to stay away from fatty and or rich foods so had an idea my gall bladder was not great but never would have believe it would cause me the pain it did for so many years, before antibiotics. On several pain groups, I kept reading about Milk Thistle improving their liver enzymes, when they had Hepatitis and or elevated enzymes. So after the gall bladder removed, I started taking Bentyl, the surgeon prescribed. Later, I got some Simply Milk Thistle by EmzymanticTherapy and I take 2 of the small capsules 3 times a day with the larger doses of medications and supplements. If I leve it off, I will have discomfort borderin on pain, but I can take the Bentyl too. My husband not taking the Simply Milk Thistle too, we figure it can't hurt and doctor okayed. There are different combinations, that is why I get the Simply Milk Thistle, nothing else really in there, and it doesn't contain preservatives, etc. There is some carob used in it and a few other things, but none bad and I have such reactions to " fillers & binders " that I have to pay the higher price most of the time for brand name due to the generics having more of the kind that people tend to react to. The bottle I buy says that it's formulated in accordance with the German Commission E. For what it's worth. The part on pain was very informative. I was under the impression that the liver contained no pain sensors and what we felt was surrounding tissue aggravated by the liver sweling up so maybe i was partly right and partly wrong but this explains it very well. To find out if every thing is normal at the liver cells level, I would consider a liver biopsy.
