Guest guest Posted March 14, 2006 Report Share Posted March 14, 2006 I am sure no expert, but my understanding is that the measurement of " free " testosterone is very telling, and your blood work definately shows that particular count to be low. I assume your doctor will make the usual decision and put you on one package of Andgrogel a day. It is standard proceedure to be very cautious with initial prescription. This is a skin spread on that will deliver 5mg of testosterone and will probably raise your T level a certain amount. Personally, I think based of your quite low testosterone level, that it won't do you much good. You may help big time in my opinion. If I was your doc I would give you a shot in your but of one CC of Depotestosterone to get you jump started and then teach you to self inject 100mg or one half a CC every eight days or so. Many members of this group agree this is the best way to handle TRT. Many have not, and maybe not you also, responded to skin spreadons like Androgel or Testim, and they are the MOST expensive form of TRT there is, next to maybe pellet implementation. Your values should be 600-800 ng/dl for total T. I am not knowledgable on the thyroid stuff but I can tell you increasing your T will more than likely help. It looks like what you have done with taking T4 and T3 has had minimal affect so why not try TRT. One word of caution is it can impact fertility if that is still an issue for you. Also, TRT is for life so don't make a decision in haste. One thing that my help you decide is do you still have a reasonable libido, do you get erections at night/morning? Are you able to maintain erections? These are all symptoms of low T in addition to being lethargic, brain fog, lack of energy and stamina. If these apply to you, then I would investigate TRT. The 1st 1 told me that I did have a thyroid problem, he had me continue taking T4 75mcg and he added T3 (Cytomel) 5mcg to the mix. THrough all of this, I'd say that when I started taking T4 for the 1st time back in November I had a nice response to it and I felt great for about 10 days and then I crashed. And then when I added the T3 I had nice response which was also prett short lived. On a side note, I've had similar reactions to stimulants over the years, where I'll feel great, " Normal " , almost instantaneously after I start them, that state of mind will last for a certain amount of time, and then I'll crash and the meds won't have any further benefit for me. I've always thought that was really strange. B/c it shows me that everything is still functional and accessible, yet I'm so far removed from all my feelings and functionality 99.9% of the time. The next Endo I saw about a month ago looked at all my blood tests and he told me that he doesn't think there's much of a thyroid issue. He said that there may have been at 1 time, but he said that if you're on the right dose for a certain amount of time and you're still not feeling well, then the symptoms most likely aren't b/c of the thyroid. So he said that he was going to run a whole bunch of other tests on me to see if he could find anything metabolically wrong with me. When the tests came back a couple of weeks ago he called me and said everythin looked normal, EXCEPT, that my Testosterone looked a little low. He said that he wasn't really too concerned about that b/c it can fluctuate a lot, and he asked me to have the blood test re-done early in the morning 2 hours after eating a high-carb breakfast, which he said will give an accurate reading. So, the Dr. hasn't called me yet with any follow-up diagnosis, but I did get a copy of the labs, and here are the results. I also have some results from a 24 hour urine collection analysis that was done last week which I'll post. Finally, I'll also post the results from the 1st set of labs that were done a few weeks ago. Guest guest Posted March 14, 2006 Report Share Posted March 14, 2006 Sounds like your primary concern is Depression...and with that probably comes a load of other issues/symptoms like low libido, erection problems, weight gain, etc...stuff you probably don't realize as the depression looms over you. More than likely it's your T levels that has set this condition over you. Would have been useful for you if you had the blood drawn when you started feeling this way and prior to starting the AntiD meds. The meds may have created your existing lab work results...or may not even have touched them one bit. Sounds like the Endo is an idiot when he says ur T levels are normal. Again, I still haven't heard back from the Dr. regarding these labs, I just picked up a copy and was too impatient to wait for him to call. If this is a serious finding I want to know NOW. I actually think it's a lot of nerve that these Docs charge all this $, they're clearly dealing with very sensitive issues for all of us, and they have the nerve to not sit on these results until it's convenient for them to get in touch with you. Hopefully this Endo is competent and will be on the same page as all of you who are clearly very knowledgable and practical. To that end, is an Endo the best typ of Doc to pursue treatment with? Does anyone have any reccomendations for Docs in New York? As far as my symptoms and my reflections on some things you guys said...I'm a young guy with no kids yet so the thought that this could mess with my fertility is a little disconcerting. With that said, based on the way I function these days I've ruled out the possibility of having kids until/unless my life somehow gets back to normal. So I guess I'm in a loose-loose situation if that's what it comes down to. Are you definitely deemed infertile if you pursue this treatment, are there ways to still have children, like freezing sperm b4 starting or something like that? Interstingly, or maybe not?, I don't really have problems getting or maintaining an erection. My problem is that I always feel so miserable w/ what y'all call brain-fog I guess. I always feel like something is " off " . I'm not interested in doing anything, that includes having sex. But when I do have sex all the systems seem to work. They treat diabetes more than anything else. Sperm banking is always an option. With your results you should consider hcg as your treatment. HCG doesn't hurt sperm production the way testosterone does. Treatments raise testosterone levels fairly quickly. How long it takes for symptoms to abate varies widely. But as time went on I could no longer push my self. To make a long story short I was on sick leave and after much testing the Dr.'s told me I am suffering from Major Depression and I have to go along with this to keep my job and get my sick pay. I had to see a Psychiatrist and go into Therapy 2 days a week and was put on every dam kind of AD drug on the market. I went though this shit for 5 yrs. all drugged up and walking around in a fog. Never felt better. Prozac came out I was put on it and could not void good and got a bad bladder infection and was sent to see a Uro. This Dr. checked me up and down front to back told me to get off all these AD drugs I have low Testosterone. Hell I had to go into a re-hab unit for 30 days to get off all the shit they had me on. Anyway going on TRT I was back to work in less then 4 months. Never was suffering from depression. Your LH and FSH is dam low meaning your pituitary gland in your brain is not sending the LH and FSH messages to your testis to make testosterone. Do not and I mean do not go on any T meds until your Dr. finds out way your levels are so low. You need a MRI on your pituitary to see if you have a tumor of some thing wrong this is not life treating and and treatable so don't get up set. Your Total T is dam low for testing in the morning and your Free is below normal. I feel your Dr. should test your Estradiol " E2 " also if high this can keep your free T down. If the brain sees your cells full of Estradiol it will think your testosterone is ok because the brain can't tell the difference thus your LH and FSH low. Do this test before getting an MRI done. Today I feel the best I ever felt in all the 22 yrs I have been on TRT I am 62. When you read the TRT in this is the tests that needed to be done cross check them to what your Dr. is doing and I don't feel you need to say anything to your Dr. about what you read at the site. Endo's don't like it when they are shown some ones work. If you feel this Dr. is not doing a good job then see if your can show your family Dr. the treatment by Dr. and if he will treat you keeping your T levels up and your E2 down and adding HCG to keep your testis working. If he is not comfortable treating you ask him to treat you though Dr. over the phone Dr. dose this. The 1st 1 told me that I did have a thyroid problem, he had me continue taking T4 75mcg and he added T3 (Cytomel) 5mcg to the mix. When the tests came back a couple of weeks ago he called me and said everythin looked normal, EXCEPT, that my Testosterone looked a little low. He said that he wasn't really too concerned about that b/c it can fluctuate a lot, and he asked me to have the blood test re-done early in the morning 2 hours after eating a high-carb breakfast, which he said will give an accurate reading. His receptionist just called me back and told me that the Dr. now wants me to have yet ANOTHER blood test doen--this time he wants me to fast opvernight and screen for " Metanephrine " and " Nora-metanephrine " . Of course I got no input on any previous results, now I guess I'll have to wait another 2 weeks for them to get these results and hopefully find out something. Once you start on T meds it is dam hard to figure out way your low and what the cause is. Looks to me your in good hands. A metanephrine test will rule out organic adrenal problems. Don't be surprised if he wants an MRI too. IMO he's doing a good, thorough test regime. That will save time and uncertainty in the long run. To many of us had docs who took shortcuts, starting treatment without all the facts. I regularly have my test results faxed or mailed to me when they become available. Your doc's staff should cooperate.
