Although your tale may feel unique, it is not in a way - you have experienced poor medical treatment and care from day 1. 250-350 is NOT appropriate level for someone your age (at 34 - I am 38) and you are seeing the symptoms of hypogonadism - poor concentration, no libido, no memory, fatigue/energy issues, depression, anxiety, etc. You need to try to find a good doctor who you trust in this area of special care (many here are doubtful of endos., but I am not sure who else to suggest) and have them run a full battery of hormonal tests - LH, FSH, E2, T4, T3, TSH, SHBG, Free T, Total T, Prolactin, possibly MRI of pituitary, if needed, just to start. When you get these results, re-post. It also sounds like you do have some financial and marital concerns, as well, perhaps driving some of the depression. I would remain under doctor's care for this - especially given the suicidal thoughts during times of depression - this truly needs monitoring by a professional and if ever you feel this way, please call your doc. or check yourself into an ER. I hope you will find relief - equipping yourself with information, a good doctor for low T, care for depressive episodes if reoccuring and a commitment to solve this issue - will help you tremendously. Guest guest Posted July 22, 2006 Report Share Posted July 22, 2006 Part 2....( sorry too long ) At that point, I am not taking Testosterone, only 15 mg of Remeron sometimes to sleep....Ambien others, but I still couldn't sleep on my own....Without any meds at all, my symptoms were this: No motivation, apathy, no energy, insomnia, memory probs, but at least " stable " Another testosterone test at this time reveals me to be about 315....never tested above that during all tests....but the other measure....Free or Total, was high...the smaller number I want to say 19. Nothing changes at this time...my mood is always the same, not good in the morning, 4-5 PM it gets better...not great, but better...at least I start doing very small things I enjoy like watch movies, play games with my kids (video games as I didnt really have any energy) Mid march....not on the Remeron at all...only AmbienCR for sleep. It works, but I only sleep 6 hours max. But still...I used to fight for 3-4 hours and the 6 is better than never sleeping. From that point I have to get a job, couldn't just rot away, and I was hoping the routine would help and honestly, we needed the money badly....I was a burden on my family and wife and Never in my life had been like that. I got work in sales ( my whole work history was in sales ) I had to put on a fake smile and 'act' happy at work to pass myself off, and even felt semi-happy at times, but mainly couldn't wait for the day to end so I could come home and wish things would change. I'm still employed there and do a decent job, but I am not half the man I used to be prior to April last year. I was always happy, uplifting, and positive mood continuously. I still only have 50% an erection, and can't sustain it for long at all. On rare occasions, I actually 'want' sex, but usually do it out of habit. My mood is good enough to pass off to everyone that I am just a dull guy. My memory is terrible...I have to write down things all day or I forget. I never had any of those issues before. And it was litterally overnight that they changed. I have no idea what my testosterone level was before April last year.....Thats horrible that males don't have that charted at least once per 5 years when growing up, if only for a reference point. I think that my vocal chords are thinner....I was in a band and used to sing, and cant' hit the lower keys I used to, and also my 'falsetto' voice is almost totally gone. If I don't take anything or change anything...I think I could function for the rest of my life, but I have no zest for life, I dont look forward to anything, I just exist. I want my old self back, to have spontaneous sex, and be able to perform in it....to have a good memory/concentration....to be happy instead of 'blah' To participate in my family life again instead of go to work, come home, sit a bit, take a sleeping pill, repeat. Does anything sound familiar to any of you? Please give some advice. I am going to my doctor this week to get a refill on my Ambien and I am going to request another Testosterone test. If it is in the 300ish range, does anyone live normally at that level? I know that this is not normal for me though....I hate to think that I may never get back to normal. My theory is that when I took the Testim, it was like a catalyst for something, and when I stopped, my system crashed....maybe Testim was wrong for me. Maybe I interrupted some sort of process with it. I just don't know. But honestly....prior to taking the Testim, my only symptom was the erection prob. After taking that, quitting it in 4 days, and until now....my life has never been the same. I never, ever had a single depressed feeling in my body, ever prior to that. I just don't believe that I am suffering from depression as I have friends/family that have been and didnt' suffer like I have. I am a big proponent of university medical centers - have your tried networking through the UT Memphis Medical Center and the Reproductive Endocrinology unit (Google helped me - I don't know TN)?? Might be a good place to ask who the doctor is who treats (and researches and trains young residents/med. school students) the majority of men with this issues?? Most certainly, stay away from PCP or general practitioners.... I want to add something....I never put in my story that my resting heartbeat has gone from mid 60's to 80+....lot of time mid 90's and that is constant...Before the first Testim dose, it was never high, and now always is. Is this an effect of too much Estradiol, or is there another possibility that comes to mind. Also, the 'nervous' feeling is constant. I never stop shaking...sometimes less that others, and particularly less at night. I know I am posting lots of questions, but please bear with me... To help you rest try doing this 2 x's a day it takes a little time to learn this but when you do you can do it on a bus or in the john at work. It gives your body and mind a deep rest better then a good nights sleep and if you do this laying down it will put you to sleep. The best thing you can do is try to relax I know this is hard to do when you feel the way you do but like anything it takes work. http://www.ucop.edu/humres/eap/relaxationrespone.html Learning how to do this is life saving and should be a part of your life from now on. It saved my life. I have panic attacks and doing this will stop one dead. Your estradiol can be to high and this will make your heart beat faster. Get some Zinc/Copper this helps keep E's in check a little take 2 pill for the first 2 weeks then just one a day. He has a somewhat different protocal than some of the suggestions on this board but he will work you through your problem. If you have bood work from previous Drs take it with you. Therefore, read the responses with an open mind and use this information when consulting a Dr. Here is my 2 cents: First I think you have a complex of problems of which one is low T. You had what I would call a significant reaction to the T you tried with Testim. As Phil and others have mentioned, you really need the test that Phil said to find out what all is happening. Low T is probably just a part of it. You can't get and keep erections without enough T. Second - My guess is your estradiol is high and when you added the testim, it caused it initially to increase (causing the hot flashes and nervousness). Find out what your baseline is and if it is > 30 you may need an aromatase inhibitor like arimidex. Third - It has been my experience that this field is rare so the average Dr. does not have a lot of experience. Therefore they go with standard practice which isn't always the best course of action. Lastly - you have take the best step you can by joining this forum. There are tons of experience and men of compassion here. Use it to discuss any topic - most of us have been there. You have a lot of stressful situations going on and you can't keep them in or dwell on them - that means talking about them. Do what you have to - counselor, best friend, or us but don't get so far down and by all means, if you get to the end of your rope, go to the ER - suicide is never the answer. I'm still employed there and do a decent job, but I am not half the man I used to be prior to April last year. I was always happy, uplifting, and positive mood continuously. I still only have 50% an erection, and can't sustain it for long at all. On rare occasions, I actually 'want' sex, but usually do it out of habit. My mood is good enough to pass off to everyone that I am just a dull guy. My memory is terrible...I have to write down things all day or I forget. I never had any of those issues before. And it was litterally overnight that they changed. I have no idea what my testosterone level was before April last year.....Thats horrible that males don't have that charted at least once per 5 years when growing up, if only for a reference point. I think that my vocal chords are thinner....I was in a band and used to sing, and cant' hit the lower keys I used to, and also my 'falsetto' voice is almost totally gone. If I don't take anything or change anything...I think I could function for the rest of my life, but I have no zest for life, I dont look forward to anything, I just exist. I want my old self back, to have spontaneous sex, and be able to perform in it....to have a good memory/concentration....to be happy instead of 'blah' To participate in my family life again instead of go to work, come home, sit a bit, take a sleeping pill, repeat. Does anything sound familiar to any of you? Please give some advice. I am going to my doctor this week to get a refill on my Ambien and I am going to request another Testosterone test. If it is in the 300ish range, does anyone live normally at that level? I know that this is not normal for me though....I hate to think that I may never get back to normal. My theory is that when I took the Testim, it was like a catalyst for something, and when I stopped, my system crashed....maybe Testim was wrong for me. Maybe I interrupted some sort of process with it. I just don't know. But honestly....prior to taking the Testim, my only symptom was the erection prob. After taking that, quitting it in 4 days, and until now....my life has never been the same. I never, ever had a single depressed feeling in my body, ever prior to that. I just don't believe that I am suffering from depression as I have friends/family that have been and didnt' suffer like I have. I feel that I am willing to try anything to get close to normal. I have another question though...Wednessday I am going back to try to get bloodwork done, and I just started the Mirtazapine (remeron) for sleep again. I only took one 30mg dose last night...it gives me a bad sleep hangover. Will it affect my baseline estradiol levels? It has a 'calming' tranquillic effect. Makes me move slower...muscles feel weak. Jittery, but the sleep is paramount to me. I will discontinue tonight and resume AmbienCR if you guys feel that it will affect my blood levels. When I stopped the medicine, it stopped as well. I used to be on 20 mgs. Paxil (which ironically I didn't take for depression, but fatigue - without a low T diagnosis!!!), but am now down to just 5 mgs. per day of Prozac. I have come to realize that my issues were never depression and anxiety as a root, but a symtom of low testosterone. Be graceful with yourself as you start and stop these meds and at all times, do it under the care and progress of a doctor, pref. a psychiatrist. Yes, I put myself on the zoloft and remeron because I had the prescriptions unfilled and just had them filled. I am going to discontinue the Mirtazapine (remeron) and the zoloft, as I am not really doing any better as a result of them. The ambiencr is a must, though. My 'average' state, without taking anything at all, is this: Shakey, jittery as if nervous, sleepy (but completely unable to sleep) tired, confusion, memory loss, no motivation, erectile dysfunction, severly depressed mood, fast heartbeat, and weak feeling. If I don't take the ambiencr...I won't sleep. I know that taking these meds without instruction isn't really the wisest choice, but after a year of suffering, I just have to find some kind of middle ground. My intention is to quit everything but Ambien and pray that I can get my doctor to do the complete bloodwork this Wednessday. These were from the endocrinologist who said that I was in the 'normal' range and didn't think I needed the therapy. 11/23/2005 Serum Testosterone 305 ng/dl Free Testosterone 15.07 ng/dl %free Testosterone 4.94 % (high flag) Amenorrhea Profile LH 3.9 mIU/mL FSH 2.0 mIU/mL Prolactin 8.2 ng/mL Then the next test was taken on 12/13/05 Serum Testosterone 433 ng/dl Free Testosterone 20.52 ng/dl %free Testosterone 4.74 % (high flag again ) TSH 1.193 uIU/mL Prolactin 7.0 ng/mL The next lab I have is only a serum Testosterone level on 3/05/2006 339 ng/dl Given my symptoms, including the shaky feelings, insomnia, depression, etc. does this help anyone see a pattern? And even readings near the bottom of the lab scale that are accompanied with " depression " symptoms and sex drive failure in particular rate a serious look at trying HRT to see if it works. But... I'm not a doctor. Check out the average age vrs testosterone chart here and download the AACE guidelines for hypogonadism. You need to be on the safe side and do all the testing to see if anything is doing this to your levels. If nothing shows up then you need to go on T meds and the best thing for you if you don't have a health care plain that pays of meds is shots get a 200 mg. shot to jump start you and do 100mg every week after. This can cause fluctuating levels of Testosterone. GEt your SHBG checked as well because looks like it is low and this causes high free T and low SHBG is usually an indication of some sort of pituitary problem mostly an overgrowth on it that starts to secrete too much of either prolactin or GH and GH leads to high IGF 1. Keep in mind that I do recall having poorer motivation and definately the erection problems since last April, but other than that...only my bloodwork revealing Low Total Testosterone (240 ng/dl) and High LDL cholestoral...revealed anything to me out of the ordinary. The day I went in and the doctor explained what happens with Low T and the possible negative effects that would happen in time, was the first I ever heard of it. I didn't have health insurance and he gave me a coupon for a free month of Testim. I went home, read the instructions, took it the next morning (5gm) and boom....all of these symptoms started. Assume that I was having a slowly declining testosterone output, or that I was not low at all, but had a misdiagnosis. What could taking the Testim for 4 days, and on the final day taking a single Lexapro (because I started having all these 'feelings' and the dr said I needed the anti-d), do to a 'normal' leveled male, or one with a slowly declining testosterone level? I know that there are too many variables, but I am just asking for theories. The result of that experience was a person with the constant shakes, elevated heartbeat, memory loss, poor concentration, lack of motivation, depressed mood (at times severely), sleepiness, and chronic insomnia with a slightly better mood in the evenings, for some God forsaken reason. Your total t was 240 your adding the gel put more T in your body so for most men they feel great and others like you the added T sends there body into stress from the T levels going up so fast. Now after a time like 5 days you brain will see the added T in the blood and stop sending the messages to your Testis to make more T because you have more in your blood. When this happens your body or base line level of 240 is no more. So now your going back down and if the dose of the gel your on is not high enough you can go back down to less then you base level. Making you feel worse so for you this had to have happen in the first days you started on it. So if your not on the gel any more don't go back on it until you do more testing. 1. To be sure your low. 2. To find out why your low. 3. If you find your low and don't know why then go on shots cost is a lot less then gels. The bottom line is to find out why your low if you are. If you do shots start at 200mgs for the first shot to jump start you then do 100mgs everyweek after. The advice given to see a Reproductive Endocrinologist is the specialist I also expect will give you the best help. Ask if the specialist treats Andorpause. Couples with fertility problems need treatment from a specialist that understands the hormone problems men can have and their treatment. After saying that, there are good and bad there too. I suggest you read all you can on the subject of TRT at and Noble. You must protect yourself from those that are uninformed. Dr. Gambrell of Augusta GA IMHO is the best I have been able to find and has decades of experience treating andropause. She said that regardless of why, I am in a state of hormonal imbalance yes, but it was caused by the taking of the Tesim in her opinion. I do agree that maybe taking the Testim 5gms did something to my body...interupted some kind of normal activity, and maybe I didn't recover to normal after stopping suddenly after 4 days application. I do remember though, the morning I was supposed to apply it...would have been the 5th day, I felt like Hell and tortured all day...until evening, and I instantly felt 'normal' for the duration of that night. The next day, I felt bad again. I'm thinking that maybe my testosterone levels at 'normal' could be between the lower 300ish range that I produce on my own, and 600 that 5mg of testosterone gel will provide, and that I was coming back down to my comfortable level that night. Then, not knowing at the time that taking the gels would interupt my own production, and that stopping suddenly would result in a Testosterone crash, I in fact crashed. After which, I have been on sleeping pills ever since because I can't seem to sleep on my own anymore. Not to mention the 'nervous' feeling I constantly have, mood problems, concentration problems, memory problems, lethargy, no motivation, half erections, etc. I asked her if my tests come back the same, ie. testosterone around 300ish, and no flags on the thyroid or cortisol, and I agree to the Lithium/antiD treatment, then if I could possibly go on Androgel (provided my insurance will pay for it) and take 3 squirts (one short of a 5mg dose) because it might be the level closest to my natural level...All my life I have not really had the most manly features, no deep voice, smaller penis, scarce body/facial hair, abdominal fat...but never had the 'depression' that I now have. I wish I just knew my standard Testosterone level without anything interfering prior to last year when I felt normal. I know I post long ones... She wouldn't give me all of the tests I asked for, if you recall, but did a few that were interesting. I am going to post the results with the name of the test, my result, and the range in that order: Thyroid Panel 3 T4(thyroxine) 7.7 ug/dl 4.5-12.0 ug/dl T3 uptake 35.0 24-45 % free thyroxine index 2.7 1.1-4.4 TSH .872 0.350-5.500 uIU/ml Cortisol Level 15 ug/dl adult am 4-22 pm 3-17 (test was taken 2:40pm) Free T3 2.8 2.3-4.2 pg/ml Free T4 1.19 0.71-1.85 ng/dl Total Testosterone 214 280-1070 ng/dl After this, she seemed to agree that I was indeed having a testosterone problem. She even agreed to the Estradiol test, having said that it was unecessary before. She took blood for that test, but I forgot the point that it was the sesitive assay for males.. I hope that it is accurate. She recommended to refer me to a urologist that she knows who treats low testosterone males. I am going to be sure to call him myself to make certain, but it seems that she is acknowledging that I am having a testosterone prob of some sort at the root of my problems. Phil, she even said that she would be willing to do an over-the-phone consultation with the doctor you recommend. She wanted me to find out the cost involved with that because I would be responsible for it as she said insurance would probably not pay it. The talk of manic depressant and taking lithium wasn't really gone into after that. Please give me your input guys on these test results....keep in mind I am a 34 year old male. Post it the same as you did here. I feel you will be better off working with Dr. over the phone send him an Email and tell him Phil sent you. I know he does not do insurance and if I were to go see him I think he would spend over 2hrs with me for the first visit a $200 and each visit after is $50 hell my Co-pay to see my Dr. is more. If your Thyroid is low best to treat this first before TRT your levels can go up when your Thyroid is balanced if it is bad. I am going to post the results with the name of the test, my result, and the range in that order: Thyroid Panel 3 T4(thyroxine) 7.7 ug/dl 4.5-12.0 ug/dl T3 uptake 35.0 24-45 % free thyroxine index 2.7 1.1-4.4 TSH .872 0.350-5.500 uIU/ml Cortisol Level 15 ug/dl adult am 4-22 pm 3-17 (test was taken 2:40pm) Free T3 2.8 2.3-4.2 pg/ml Free T4 1.19 0.71-1.85 ng/dl Total Testosterone 214 280-1070 ng/dl After this, she seemed to agree that I was indeed having a testosterone problem. She even agreed to the Estradiol test, having said that it was unecessary before. She took blood for that test, but I forgot the point that it was the sesitive assay for males.. I hope that it is accurate. She recommended to refer me to a urologist that she knows who treats low testosterone males. I am going to be sure to call him myself to make certain, but it seems that she is acknowledging that I am having a testosterone prob of some sort at the root of my problems. Phil, she even said that she would be willing to do an over-the-phone consultation with the doctor you recommend. She wanted me to find out the cost involved with that because I would be responsible for it as she said insurance would probably not pay it. The talk of manic depressant and taking lithium wasn't really gone into after that. Please give me your input guys on these test results....keep in mind I am a 34 year old male. At one point my total testosterone was over 400, but all the other tests have been less than that. Here they are: 11/23/05 Serum Testosterone 305 ng/dl Free Testosterone 15.07 ng/dl %free Testosterone 4.94 % (high flag) Amenorrhea Profile LH 3.9 mIU/mL FSH 2.0 mIU/mL Prolactin 8.2 ng/mL Then the next test was taken on 12/13/05 Serum Testosterone 433 ng/dl Free Testosterone 20.52 ng/dl %free Testosterone 4.74 % (high flag again ) TSH 1.193 uIU/mL Prolactin 7.0 ng/mL Again, after these tests, my next test result with my usual doctor showed me at a testosterone level of 338, and my most recent at 214. My symptoms again are Insomnia (the one I am most concerned about), depression, concentration, memory, aggrivation, lethargy, elevated heartbeat, jittery/nervous, motivation.
