You are asking the right questions and taking the right steps. I ABSOLUTELY believe that you need to find another doctor. While it sounds like your doctor is " sort-of OK " , he is far from aggressive and does not listen to your continued issues. You should not have suffered this long. Please don't self-medicate but find quality care, not matter the distance. You did the right thing going on the Androgel and I think the questions below are typical and usual response to Androgel and stopping of the LH/FSH/T production in self. Start asking around and looking today for the best endo. doctor you possibly can find with experience in Hypogonadism - do you know if you are primary or secondary? All Endos are NOT created equally. When you present yourself to a new doctor, go " light " on bringng up/documenting depression issues, unless they are truly significant medical concern to you. I have found if you tell a doctor or endo. that you have experienced depression, anxiety, suicidal thoughts or panic in the past and/or have been on or are on medication, the docs are VERY QUICK to label you as depressive first and hyogon. second - when treatment doesn't work, they always blame failure on your " depressive state " or " depression medications " . Don't take this as an answer ever, as depression in most cases is an result, but not the root. You perhaps should work with a psychiartist too who can work with the endo. If you feel you have hypogonadism, get into it, get all the test, get the very best care you can find and start to address your limbo siutation. Here is my opinion based on what I have read so take them for what you think its worth - Only you should make a decision to change: First it seems to me you are somewhat anxious about this which is understandable. I think some of your symptoms may be psycological and whether we want to admit it, it plays a big role in how we percive things. An example of this was I had ED for so long that I was sure I wouldn't be able to keep an erection even on TRT. I was able to use cialis as a crutch somewhat but it was really a mind game on my part. The whole reason for all the above is we have to be ready for this and it sounds like you are there. The dull pain in the groin is common as the T is causing changes to your testicles and this area if full of receptors for T. I think the other symptoms you described are anxiety related. You are going to have to keep telling yourself that increasing the amount of T will feel different. Its not what you are use to. Secondly, if for some unusual reason you get to feeling out of control, discuss this with someone ahead of time and be prepared to call them no matter what the time. If you get to feeling suicidal again (which I do not expect) call them immediately and get help. Take it day by day and it will not be something that will correct itself overnight. It took me over a year to get close. Keep us informed. Based on your results from 5 gms of androgel, which is usually not enough for a lot of us, go slowly in increments and don't change too quickly. Most changes take ~ 4-6 weeks to stabilize so I would only recommend making a change in 4-6 week increments. I know you want to get there but it is also important to not overshoot which can cause a whole other set of problems. I feel you are Secondary or your E2 is so high your brain is not sending the LH and FSH to your testis to make more T. You see the brain can't tell the difference between E2 and T so if your high on E2 it thinks it's T. Your Dr. is wrong to not test this. I went through the same crap your going through I was treated for Major Depression on every kind of AD drug on the market for over 5 yrs. before they found the problem was not depression but low T. You started on 5 g's of gel keep this up for 2 weeks and get tested if your levels are not up to the upper 1/3 of your labs range for a young man the up the dose. Most do much better at 10g's. I too crashed on my first gel when I first started TRT, I crashed after 4 days too. I too have low SHBG. And I also have thyroid TSH on the low side. I to have a similar endo story. Whenever I start any form of TRT I feel good and then the positives die off after a short period of time. With me the problem is not E2 and has been a nightmare to ascertain with no TRT actually working for me. So then it is usual to have these kinds of reactions when starting regular TRT...the small bursts of 'adrenaline' or heart racing, the dull pain in testicles, and a bit shaky and they usually subside after a while? I am going to have another total T test done asap so what is best time/place for blood drawn if I have been on Androgel and I usually apply at the bicep/tricep upper shoulders area? I feel you are right , and I honestly feel that if I could get a doctor that is interested enough in me, they would find that I would do well if not best on some kind of E2 lowering drug instead of outright TRT, as my testicles work, I just either don't put out enough signal or something is prohibiting my LH production. My testicles are both regular sized and firm right now. My testosterone is low...along with LH/FSH on the lowest side of normal ranges, so if the LH signal could be boosted in some way, maybe my testicles are capable of producing the level of T that I should be making. Its hard to believe that here in Memphis, TN, I have trouble finding a doctor with good experience with this and my endo says, " We don't test for estradiol (estrogen) in males unless they display enlarged breast " " The way we treat low T is to give T...not to use a SERM " God Bless, Chris God Bless. Get the Test first then when you get back home put on the gel. If you add HCG this acts like LH and will keep your Testis working the best they can. Dr. will work with your Dr. to treat you over the phone ask your Dr. if he will do this. But not the Endo ask your family Dr. Go to www.allthingsmale.com and Read his site plus TRT: A Recipe for success and the HCG update. If this dose not work go to the files section at the home page on the left and click on Finding a Male Hormone Dr. this will help you to find a new Dr. When your on TRT your LH and FSH will go very low and stay there because you adding T to your body. The brain sees this and stops sending the LH and FSH to your body. So then it is usual to have these kinds of reactions when starting regular TRT...the small bursts of 'adrenaline' or heart racing, the dull pain in testicles, and a bit shaky and they usually subside after a while? I am going to have another total T test done asap so what is best time/place for blood drawn if I have been on Androgel and I usually apply at the bicep/tricep upper shoulders area? I feel you are right , and I honestly feel that if I could get a doctor that is interested enough in me, they would find that I would do well if not best on some kind of E2 lowering drug instead of outright TRT, as my testicles work, I just either don't put out enough signal or something is prohibiting my LH production. My testicles are both regular sized and firm right now. My testosterone is low...along with LH/FSH on the lowest side of normal ranges, so if the LH signal could be boosted in some way, maybe my testicles are capable of producing the level of T that I should be making. All it means is your Pit is not working good enough to get your Testis to make enough T. You do need to get on HCG later when your levels come up and your stable. Some men that are Secondary only do HCG. You need to add this to keep your testis working. Now that you know you have a Pituitary problem it's time to test other things like DHEA-S, Cortisol, IGF-1, Thyroid and Glucose. Your Pit may not be sending to these areas also. Do a search on Pituitary +low Testosterone and read up on this. With gels your putting a new amount into the blood eveyday with shots it's in oil and most of the shot is felt after 2 days then your levels go down very slow this is why we do shots every week now. They use to do them every 2 to 4 weeks. Guest guest Posted November 17, 2006 Report Share Posted November 17, 2006 She gave you a 200mg shot are you going to do this every 2 weeks I feel you first shot should be 200mgs to get you started then do 100mgs. or .5mls every week. I don't pinch up the skin or shoot in the top of my thigh. I use a dam small needle 27g 1ml x 1/2 " lg. the T come out of the vile real slow so I pull the plunger down all the way and hold it. It takes about a min. to get a .5ml dose out I pull out more then .5mls then push it back into the vile to get the air out. I shoot off to the side of my thigh I bury the needle into my leg and slowly push the T into my leg. For my next shot I do the other leg. It reads Testosterone CYP 200MG/ML 10ML Qty " if it says this on the vile your 1ml shot is = to 200mgs. so to do a 100mg. shot you only need to pull out a .5ml. Do the math and it will come to you. You said the Vile says 200mgs./ml for a total of 10mls this is a double strength or Two times stronger the the 100mgs/ml vile. So to do a 100mg. shot using a 100mgs/ml T vile you pull our 1ml. But to do a 100mg. shot using a 200mgs/ml vile you pull out one half that amount .5mls. The dose of your shot is typed on the slip then glued to the box. But the box is not 100mgs./ml it's 200mgs/ml. If our still not understanding this call Wal-Mart and ask how much you need to pull out of the vile to do a 100mg. shot. It sounds as if Walgreens has made a mistake on their pamphlet. I would contact them and get a clarification for peace of mind. Some are doing test shots subqutaneous, but I am going to consider it in the experimental phase. I would suggest that you do the shot into the muscle. That is the tried and proven method.
