Back From The Rheumy

Well I found out why I am getting a CT scan.

(thanks for the replies).

The x-ray technicians are seeing something in my left knee that is

not PA related and is nothing to worry about...(yeah right)

Anyways I have been going downhill in a bad way with my PA recently.

I told my rheumy on Tuesday that I hurt bad. I really wanted to start

a DMARD because I am young(22), but he is a older rhuemy and is very

traditional in arthritis treatments. Start off small and go bigger.

I told him my histroy...

Celebrex --- worked great for a year, then stopped

Vioxx (50mg)--- worked great for half a year, then stopped

His next step is to put me on Volturen....

I have never heard of this drug??! Anyways he said it is a older drug

and may work. I have a 6 week checkup with the same doctor. If the

Volturen doesn't work he will put me on MTX.

Moderator 2 cents: I was on Celebrex until I ended up going to the Emergency

room because of upper respiratory problems due to the Celebrex. Tried Vioxx for

a brief spell and it wasn't working. Went on Voltaren, actually Diclofenac is

the generic and it has worked quite well for a whiile now. Good luck, PatB

Hi Shane,

The Voltaren/Diclofenac is a NSAID. There are several. Some work

for some folks, while others work better for some. Try as many as

you want until or if you find one that works for you. Preventing

damage to your joints should be a concern. If you currently have no

joint damage, try to keep it that way (NSAID's will not necessarily

prevent joint damage). I am 42, had no joint damage, tried 5 or 6

NSAIDS (including Diclofenac, none relieved my pain or reduced

swelling. So now i have been on MTX for about 10 months. For me,

10mg a week means that I have no pain,no swelling. NO SIDE EFFECTS.

Many others, do experience side effects.

It seems to be all trial & error and can be frustrating.

In my area, doctors are not too quick to prescribe percocet and oxycodin. I

need my Vicodin ES almost every night to sleep for the pain and my PCP is not

too thrilled to give it out. Do most rheumy's prescribe it without a

problem? I am going to ask him for it. I was just diagnosed several

months ago and lived with the pain for years until now when it has gotten

much worse. He is a new doctor.

Sam,

Are you on any DMARDS? My toes are a problem for me too., but not as bad as

yours yet. Does your Dr. not think a DMARD is necessary now?

what'd the doc say about the toes? I can't decide if I wanna go ahead and

make an apt with a podiatrist as well

She suggested that I get new orthotics that would

raise the ball of my foot and push the toes together

and, hopefully, ease the pain. I haven't looked into

that yet as I want to wait until this flare has

settled down a bit, but I plan to do this as soon as I

can.

I'm very blessed to have a rheumy who's #1 concern is

pain control. Until I started with this doctor, it

was difficult for me to get anything, even Tylenol 3.

There is a huge concern about addiction, when the real

concern should be pain management.

Moderator note: Rheumys tend to have to write more pain meds scripts and the

amount brings them under the scrutiny of the DEA. I get my pain meds from my

local doctor who doesn't write as many pain meds scripts which means the Rheumy

has fewer to write. Patb

I've been trying to postpone starting MTX - I actually

have the prescription filled already. Bextra is a

last resort. If it doesn't work, I have no option but

to start MTX. I've tried all the usuals -

Chloroquine, Gold, etc - with little or no success.

I'm so fearful of the side effects with MTX, which is

why I've put off starting it before now.

Sam,

I finally started the MTX and at first the side effects were horrible...I was so

tired. But after being on it for three weeks I feel much better. I still hurt

but the side effects aren't so bad since I listened to someone on the list who

said take it at night ...it really did help with the side effects.

Sam,

Bextra is solely an NSAID. It will help the inflammation, and maybe the pain,

but does NOTHING to stop progression of the disease or to stop damage. Why do

you have to go for MTX? Why can't you try Arava, Azulfidine, (there are others I

can't think of right now). I know I keep pushing this, but if you want a good

drug guide... a list going from easy on the body to hard on the body, go get the

book " Strong Women and Men Beat Arthritis " by Dr. Miriam . She lists out

all of your options from prescription drugs (with a list of side effects for

each) to supplements (with of list of myths and fact of what works). This is

where I learned that there are SO MANY other easier on the body drugs out there

than MTX for me to try before going on that. Have you tried Enbrel Sam? It's a

little scary, but people who don't have problems on it seem to do very well.

When I get the chance I will get my book and e-mail you the whole list.

That was my next step. Calling a podiatrist. My orthopedist told me to do

that because I am walking funny due to the pain. I am definitely going to

give it a try.

are they orthodics that can be prescribed by the podiatrist? or I have to

buy? This just interests me as I'm on my feet all day.
