Guest guest Posted July 2, 2004 Report Share Posted July 2, 2004 Over the last couple years (I'm 40 now), I've noticed my testicles have noticeably gotten smaller to the point that whenever I exercise my testicles are completely drawn up into my body (also happens during any sexual activity as well). Over the last year I've had to fight depression as well (which I have never had before). I continue to have erections but have a very noticeable decrease in libido. I've noticed that my testicles seem to have loss mass/weight. Has anyone here ever experience this? (I've also had changes in my ejaculate where it used to be thick and white, it is now clear and very watery). I went to a urologist 2 years ago when this started but he said it was normal and without doing any tests sent me on my way (with a lovely prostate exam, of course ((gag!)) ) Are these symptoms of just getting older or has anyone here experienced it with loss of testosterone? One had been damaged by stangulation when a teen but had 4 kids with the good one. After they stopped giving me enough hormone I wasn't upset that the therapy caused them to shrink. Considering the benefits of TRT it was the right thing to do. My endocrinologist provides all sorts of forms of TRT including the gels, patches and injectibles as some people do not want to have the pellets installed under the skin. He says he can not usually get the levels of T high enough for them to enjoy the benefits from these other alternatives and after several months, only 45% the patients continue. About 65% continue then they started using the pellets. I prefer the pellets so I don't have any daily involvement with the therapy. My testicles are now about the size of a medium sized to large grape. They used to be about the size of a small chicken egg. I know it sounds almost ludicrous to make comparisons like that. I've read on the internet that there are times when some guys have to have surgery to prevent their testicles from retracting into their abdomen. I read on this forum a lot about what people are going through AFTER they've been diagnosed but not what led to them being diagnosed or what led them to seek treatment. Your regular doctor can order basic tests (serum T and free T at a minimum). If results confirm low T, you should try to find a KNOWLEDGEABLE endocrinologist to first check for possible causes. Extremely low libido was what led to tests confirming hypogonadism. I did have an endocrinologist who measured my nuts by holding them in one hand and comparing them to a set of oval wooden beads on a string. Unsettling. I didn't make the obvious rosary joke, 'cause I'm sure he'd heard it before. Anyway, IMO you should see an endocrinologist. The trick is to find a good one. Your experience with your urologist tells you a medical specialty doesn't guarantee competence. I was diagnosed with iron overload (hemochromatosis) which caused my pituitary to shut down and my testicles to shrink. I am not having much luck getting them back up to size. I guess no speedos for me. There is another name but not important now. I have been treated by an endocrinologist in Augusta GA who has decades of experience treating andropause and couples having fertility problems. Theses grreat specialists are hard to find and you doctor may not even be aware of any with decades of experience. Most doctors now providing hormone therapy have gotten their information from the pharmaceutical reps providing the products they prescribe for their patients. This leaves out hormone pellets from Compounding Parmacies which have been around since the 50's, are reasonably priced, ideal trouble free till the next pellets are needed 4 months later. They are reasonably in cost as well when considered against the expense of the other therapies available. Dr. Gambrell, Augusta GA is one of a few remaining of the more than 100 doctors trained by Dr. Greenblatt in Georgia Medical College, Augusta GA. Back to your problem. You must find a specialist to treat you. You may still be infertile but that will come out of the testing. If you wish to have children, now may be the time to save your sperm with the advice of your doctor. I am a fitness trainer and you can be just as virile at 40 if you look after your body and exercise properly. So if you are reasonably healthy, the obvious next step is to get BLOOD TESTS... you cant tell whats happening until you get a thorough profile done of everything. Go to www.allthingsmale.com and read Dr. 's HCG Update. Now if your testis don't work well say your TT base line levels were 120 and your LH and FSH were real high time means your Primary and your testis can make enough Testosterone still adding HCG it acts like the LH and FSH message the keeps your testis working as best they can and the other LH cells in your body and brain. Men on TRT there brain sees the T in the blood and slows down sending this message of LH and FSH. So if you do shots every week do a 250 IU's shot of HCG the 2 days each before your next Test C shot. If you do Test C shots 2x's a week or every 3 days do the HCG shot 250 IU's the day before your Test C shot. One gels or creams do 250 IU's of HCG every 3 days never do more then 500 IU's in a day. I read about this and tried to get my Dr. to let me try HCG he said because I am Primary it would not work. So Dr. sent me this post to help me get my Dr. to let me try HCG. ==================================================== He probably feels that since you suffer primary hypogonadism (I am guessing) there is no use in adding HCG to your protocol. There are several reasons why this is not so. First, you have not lost all Leydig cells, so any HCG you take will stimulate those who still function to produce endogenous testosterone. This will support testicular size. We should not ignore this aesthetic consideration. Next, if he reads my work, he will learn that HPTA-suppressed (as all TRT patients are to some extent) also suffer decreased pregenenolone levels, which is the first step after CHOL in all three hormonal pathways which begin with CHOL. HCG increases pregnenolone production, and therefore restores a more natural balance of our hormones. Next, nearly all TRT patients who add in HCG to their regimens report an increased sense of well-being and also libido. These are genuine quality of life issues. Finally, I just instinctively do not want all those LH receptors (including those we have yet to discover and appreciate) unstimulated. ==================================================== My Dr. read this and let me try it he put me on 500IU's 3x's a week and after the 15 shot we did labs my labs for Total T went from 600 to 1200 so my Testis work and I am not Primary. We did an MRI on my Pituitary it was fine but before I got sick I had a head injury some 29 yrs ago and this damaged my Pituitary. It took me 23 yrs to figure out and was the reason I did not feel so great on TRT. Now I treat all my low hormones and feel better. When I first went on HCG my testis were the size of small grapes in about 4 months on HCG they came back up in size. So I was told I am Primary for 23 yrs now it's been 29 yr and in the last 6 yrs adding HCG and treating my other low hormones I feel good. DR Overbeck is one that does not care where you get it as long as it is used properly and under supervision.
