For cost efficiency and treatment effectiveness, I would recommend either: a) Weekly testosterone cypionate shots of at least 100mg per week. These can be taken as intra muscular shots in the butt area or sub q shots in the stomach fat (easier to do and just as effective). The only problem with this treatment ist hat it may cause your estrogen to go up. If that happens, you might need arimidex to help manage taht problem, and unfortunately arimidex is relatively expensive. Luckily though, you don't need much arimidex to keep your estradiol in check, so T Cyp shots with the possible addition of arimidex is deifnitely an option worth considering. Testosterone pellets. These are a few hundred dollars every four months. Might sound like a lot of money, but for something that will completely change your life in a positive way, it is worth the money. You may have very little money, but the fact of the matter is that your earning potential and employability will be much higher if you have a healthy feeling body and are sharp of mind. My endo seems to think low total testosterone is okay if the bioavailable level is in normal range. Low testosterone levels I just had my blood tested for testosterone levels. They told me it was 291 (ref 241-827). I'm scheduled to have it checked again to see if it is still low. I'm 42 years old. From what I've read, at my age it should be about 600 instead of 291. I'm a little nervous about the whole thing and am hoping that the first one was just a fluke. If it is still low, then what? I've read about HRT, but really would rather not have to do that since there seems to be many side effects that don't sound too great (lowered fertility, decrease in testicle size, etc.) I suppose my question is how safe is it? Also, is the benefit worth the risk? Are there other natural ways (that work) to increase testosterone? Anyway, I will have to wait for the results of the next test. However a man with a reading of 291 has a long way to go to get up toward 600-700 where health occurs. If he had a total T reading of 550 or 600 and his free or bioavailable were in the upper third of those ranges then there might be a reason to leave it alone but that does not appear to be the case here. Those factors help around the edges a little but you cant create T where there is none overall. I didn't buy it and that's why I contacted Dr. . Of course, this is the same endo who put me into almost-suicidal depression by undertreating my thyroid levels after I had that gland removed. They told me that the first test was only looking for total T and this new test will be looking for total T, prolactin, and LH. (It seems to me that if I have to go through the agony of having blood drawn they could at least check for everthing with the blood they have, but, they don't.) The doc I'm seeing is a Urologist, but, I'm thinking if this test comes back with low levels again, I may need to switch to an Endo and maybe he would be a little more thorough. For Philip: I read the links you sent. Very informative. I guess 291 is pretty low for a guy my age. I do have one daughter but am not sure I want to forever end my fertility by taking T. (assuming that's what happens) However, I agree that I do need to take care of myself. My symptoms mostly are the memory problems, mood swings, some fatigue/lack of ambition (but then who isn't a little tired?) I may have some loss of libido, don't have too much problem with ED and still have occasional morning erections, although not as intense at they used to be. (I don't know how much I can blame on low T or if I'm just getting old and should expect this?) I don't notice the problem so much with the sexual side of it, it's the memory and mood swings that really bother me, and was the reason I had my T checked in the first place. I read that type 2 diabetics tend to have low T. Could blood sugars cause these same problems? If I go to an Endo, would he check that possibility. (I don't have diabetes that I know of) Anyway, I won't know the results of this test til around the 9th of March, (doc's going out of town). Hopefully it will tell me that I've worried for nothing! I was not aware this could be a symptom of low T. I have this issue as well and wrote it off as being an age issue although I am 45 and tend to think that is young for memory loss issues. Did your doctor say low T can affect your memory or is this widely known and just got by me? I came across different links of studies showing a relationship between low T and difficulty with memory. At 42, I thought I was a little young for this problem. My wife had to see the Urologist, so while I was there I asked him about it and he said they would be happy to check my T levels, since that was one of the symptoms. Sure enough, it was low. And I've been dealing with this since an illness started it at age 19. The low T wasn't found until I was 30, and I didn't find a doc who really knew how to treat it until I was 32. But I have definitely seen an improvement with better TRT. The doc who finally treated it aggresively was an endo at a major teaching hospital. She was an endo that knew more than diabetes and thyroid. I think she is a member of the AACE. Unfortunately she is out of the country now and is not sure she is coming back, but she gave me a referral to another endo at the same hospital. I may be going to see him soon if my primary is not comfortable making some adjustments. I did see where he is a member of the AACE. It started for me with some sort of unknown tropical disease, probably a parasite of some sort. Diahreha close to 30 times a day for awhile on no oral intake, just IVs. They slowed it down with an antiparasitic med, and then I developed an extremely severe EBV infection with severe mono and hepatitis. Health has been up and down since then, but overall the curve has been downward. Been unable to work for almost 3 years now. I'm classified now as having CFS - my doc thinks in my case (and in most cases - he is a CFS expert and professor at the same teaching hospital) it is a chronic viral infection that they don't know how to test for yet. I just read that some of the latest research indicates that it may be a viral infection in the brain tissue that doesn't show up in the blood. That could explain my poor pituitary function. I've also have an elevated ANA, so there is possibly an autoimmune process going on as well. Hormone 101 so to speak will provide you with enough info to understand the benefits and the treatment. It is tough to find good doctors that have any experience treating men with hormone problems. You need to know when you are being treated badly so you can go find another doctor. Also, if you could expand on how overweight he is and is it just a libido issue or does he have ED problems also. I know these are sensitive issues but you are talking to a bunch of guys who have been through it all. My guess on the limited information you have given us is that two things have happened. First, he quit producing enough testosterone and became estrogen dominate. We men convert testosterone to estradiol due to aromatase which is an enzyme that becomes more elevated with the amount of fat we have. Therefore it is a vicious cycle, you produce aromatase which makes estrogen, which makes more fat which makes more aromatase....... He should have had his estradiol checked prior to starting on the cream but most doctors are not aware of this. He then overdosed on the cream which gave him too much testosterone, which then converted to estradiol. When a man has too much estradiol (E2), it has the same affect of too little T. No libido, erection problems, fatigue and added fat deposits around the waist and chest. I would have him tested for E2 and possibly look at an aromatase inhibitor like arimidex. This will stop the conversion of T to E2 and make him T dominate again. " Normal " is a very wide range so twice normal could mean almost anything. I already don't like the advice you're getting. These creams last no more than 24 hours. So applying it every other day will send your husband's levels swinging all over the place. Assuming that cutting the dose was appropriate in the first place, they should have told you to apply less of the cream but apply it every day. That way you can have stable levels. I is dam hard for me to believe his levels are 2x's as high as normal. Here is way a total T test range is 262 to 1593 for my lab what is normal would be say 600 to 800 so his Dr. is saying his levels came back at 1200 or more. No on any cream I every did and I did the strongest dose ever made. I am not saying his Dr. is not good on TRT but with out reading his tests we will not know what his Dr. is saying he could be looking at a results of 400 and be thinking normal is 200 I have seen it all with Dr.'s that don't have any idea what to do treating low T most only know what the drug reps tell them. Also, esp for gels guys generally get elevated estrogen. When did your man put on the cream and where and when did they draw the blood and from where? What brand of cream is it and what is the strenght and what is the dose? mg/mil? Most likly his " real " levels are much lower then the blood test says as they may have pulled blood from the same arm he applied the cream to. Your right I tell everyone I can never go on T meds untill all the testing possable are done. Men can have low T from so many things and I learn abou a new one everyday so think aobut that everyday. Men can have low T for many reasions High Estradiol, low Thyroid, high Thyroid, low Cortisol and high, liver problems, SSRI meds, Pituitary problems the list is so long I could be here for hours and my spelling sucks. It was a special formulation from my compounding pharmacist, at a normal concentration (5% I think). The original formulation did nothing (level in 200s) - the second formulation put my T at over 1,000, then over 2,000 on the next test. I was applying it only to my scrotum, which is highly permeable. At around the time of the second test, I ended up switching to injections, for the sake of convenience and consistency. How did you feel at the time and now doing shots what are you on and how much. I don't feel quite as good on injections, but I'm still in the process of stabilizing my levels and getting my E2 in a good spot. My E2 was 10 on a recent test, so I am really cutting back on the Arimidex (I was doing 1/2 mg twice per week - going to try 1/4 mg once per week - I'm very thin so I don't aromatize that much). I was doing 60 mg T cyp once per week, but my T level was 227 on the day of my shot (before the shot), so I have been doing 120 mg per week for a couple of weeks now and feeling better. I have also started Novarel (250 IU twice per week per Dr. ), but it does not have any effect on mood or wellbeing. My body temp is low also (about 97.4 when I awake, about 98.1 during the day). I have some thyroid and/or adrenal problems that might be causing it. I'll keep an eye on my E2 and my temp to see if there is a relationship. I do my Novarel exactly the way you do (1/2 " needle into the thigh, which is IM for me) and feel no effects whatsoever. Maybe my LH is still high (I'm primary) so adding more HCG wouldn't be that much of a change for me. Maybe one's LH would need to be low in order to feel a difference when using HCG.
