Hi Will,

Plenty of others have experienced this. Here's what could be going on:

1. Your natural T shut down after a while on the supplemental T (this is normal

and expected). It happened like this: at 1 month you were getting 300 ng/dl

from the natural T your body made, plus 550 from the Androderm, = 850. After

that natural T dropped off, so you were (are) getting only the 550 (Without

later

tests, these numbers are obviously approximate guesses.)

-and/or-

2. Supplemental T gets converted to an estrogen called estradiol, aka E2.

Over time your E2 levels built up. Your T was higher but so is your E2. High

E2 virtually negates the benefits of supplemental T.

-and-

3. Free T is that portion of T that is available to the body, the rest is bound

up

by something, especially Sex Hormone Binding Globulin (SHBG. Low

production of testosterone and increased estradiol (E2) stimulate production

of SHBG, resulting lower free T, even when total T is high.

-These are all-too-common causes of TRT failure. Too many docs merely then

conclude TRT doesn't work for a person, and let it go at that, never finding out

why.

IMO identifying and correcting the above possible problems will get you back

or close to where you were about a month after starting TRT.

Ask (demand, coerce, threaten) your uro to order tests for at least T, free T,

estradiol and total estogens. Take along a copy of the AACE Guidelines will

probably provide him with new information from a credible source. You can

get a copy in the files section here:

You're on the young side for this. Have you had an MRI to rule out a

hypothalmic or pituitary tumor? (Fairly rare and then usually benign, but it

should be checked.) Ask your uro about this.

Speaking of young, are you planning to have children? If so your next

appointment should be about finding a different therapy (dynamic stimulation

of natural T production), because TRT will eventually make you sterile. Or you

could have sperm frozen & continue TRT.

I suggest holding off the anti-depressants , see what good TRT will do. I think

you'll see enough improvement to eliminate performance anxiety &

overthinking.

You say you have never been what you consider " normal " sexually. That may

be because the hypogonadism was going on for a long time before it was

identified. Maybe since puberty.

It's quite possible that your E2 levels have climbed over that time. T

converts to E2 and DHT. High E2 levels kill libido and cause erection

and ejaculation problems.

Get an E2 test.

Hope you are still reading replies.

TRT ramps up very fast with the various TRT therapies and has 2

effects I know of.

1. I was treated with 20 75mg hormone pellets inserted under the skin.

Very soon the libido/erections are very easy for a few weeks and you

can get it back almost instantly when it drops.

Four months later I complained to the doctor that his second set of

hormone pellets did not give me the few weeks of fantastic erections I

had with the first set of pellets. I felt the pellets were not

effective the second time. He told me this improved erection effect is

temporary and due to the ramping up of the testosterone levels. I was

really ticked off.

I felt good, had a sense of well being and my mental capacity was

improved. I was getting the benefits of high normal T levels. I was

more interested in sex, but could not perform. The ED was back just as

I was experiencing before starting TRT. I was really ticked off as I

thought I had an ED cure. Had to get back to reading about ED some

more before going to Dr. Goldstein, Boston Univ. to get started on

trimix which still works great after more than 10 years. I have age

related nerve damage.

2. When you add testosterone fast, the body sometimes reacts with a

defense against something becoming out of balance. Your body regulator

senses the suddenly high T and tries to convert it to E2 to get it

back in balance. This high E2 level can have bad effects and needs to

be brought down. Hormone pellets dissolve slowly and the body doesn't

usually react by converrting T to E2.

Ernest, I appreciate the response. As I am still in my 20's, I am

hoping the #2 option is what's occuring. Blood work on Tuesday should

answer all of my questions.
