You should file your complaint with your State Department of Insurance. You

should be able to find them online and usually they will prefer you to fill

out a form of some sort. You should probably be able to download it from the

webpage or even fill it out online. The more details you can give the better.

I am the " complaint coordinator " for my office (we are a third party

administrator of health and life insurance policies) so I see all the

Department of Insurance complaints that come through here. The more detail

and documentation you can provide the easier it is for the department to

review your case.
