How do you get a prescription for B-12 shots?

I was considering enrolling my daughter in a trial reserach study at

the MIND Institute at UC (it is ongoing if anyone is interested

and they will provide 6 months of shots at the end of the study)...but

the distance prohibits us from doing so.

We have an appointment by phone with Dr. DeMio...will he write a

prescription? Our pediatrician will not.

Any advice is appreciated...many thanks friends
