Here in Florida, you still have to go down to the Health Department and get a state exemption form. The schools here will not accept a letter from a parent. Guest guest Posted September 15, 2004 Report Share Posted September 15, 2004 Yesterday the school nurse called and left a message stating that the school district didn't accept my form letter for a religous exemption, and that I will have to handwrite a letter. Give me a break!!! What difference does it make? I am sure they just want to give me a hard time because I am not getting my daughter vaccinated. But I am not sure what to write in the letter. I want to make sure I do this right so that don't turn around and refuse it and kick my daughter out of school. If you go down to the section where it says Waiver, it has some good info, including a link to an example of a religious exemption form from a University with numbers of the regulations etc. that maybe you can borrow some text from to make it sound nice and official. No problem luckily. However, I did find this form letter on Google. I hope it helps...In accordance with (your state's law reference), we hereby certify that the administration of vaccine and other immunizing agents to our child,____________________, conflicts with our sincerely held religious beliefs. We therefore require that our child be exempted from the school immunization requirements in accordance with the exemption clause found in the above mentioned section of our state's Administrative Code. We are informed of and we fully understand the risks of non- immunization for our child. We swear that all of the foregoing statements are true to the best of our information, knowledge, and belief. All other info I read for exemptions for your state said to contact the Health Dept. for a form. I typed my own letter on my personal letterhead on my computer... In accordance with Indiana Code # 20-8.1-7-9.5 section 2a: I, Nadine Turnbow, certify that the administration of vaccine & other immunizing agents to my child, Dawson Turnbow, is contrary to both my husband's & my personal religious beliefs, held either individually or jointly with others, and I therefore request that my child be exempt from the immunization requirements of the Indiana State Statute 9.5. Sincerely, Nadine Turnbow I then took two copies and had them notarized; one for school, and one for my files at home. Hope this helps!!
