edema and 4 surgeries and your history and physical

exam lists none of these that I can see "

Ok, I'm really confussed now!!

The PCP I go to now is treating me for migraines, depression, and GERD. My

surgeries were C-Sections and a Tubal ligation... do I need to get those

records and send them in as well or does that type of stuff not really

matter?..... or does it just need to be LISTED on my History and Physical by

our current PCP as part of our History? I put on my application form that I

suffer from knee pain and back pain.. and I DO!! But my doctor doesn't treat

me for it.. I take Motrin and live with it. So does that mean it won't be

counted as a problem?
