Please help me with some information like how to contact them, what paperwork needs to be done, any insurance paper work, how soon they can come and evaluate the baby etc,.I am getting desperate to get some help .The clinician at CT told me that my son had tort. Begin by telephoning the local branch office and asking for the On Duty (OD) person for an " Inquiry. " The OD collects basic information. Then the branch manager arranges an appointment for the referring persons to meet face-to-face with a service coordinator to complete an assessment for eligibility. When we had our EI evaluation (as part of California's Early Start program) two people came to our house - a Physical Therapist who physically worked with Nicolas and a Service Coordinator who talked to me and filled out the paperwork (an Infant-Toddler Developmental Assessment (IDA)). They were probably over for about 30-45 minutes. They evaluated the following: * Physical & Motor Development (Current Health Status, Vision & Hearing, Gross Motor Skills (body movement), Fine Motor Skills (hand use)) * Communication (speech & language) * Cognition/Problem Solving * Self Help A couple of weeks later I had another meeting with the Service Coordinator where she provided me with a copy of their evaluation - a two page write up by the PT and the official Individualized Family Service Plan (IFSP) which basically stated that we did not qualify for Early Start services. The IFSP summarized Nicolas' level of development in the above mentioned categories. At the time of the evaluation he was 7 months old and everything he was doing was at the 4-7 month level with the exception of Self Help where he was functioning at the 8.5-10 month level, as defined by the IDA. If I can find it I can fax a copy of the report if you are interested in seeing it. The whole process took about a month. In the meantime we were receiving PT services through our insurance company.
