We submitted our claim ourselves, and were told that CT was an out-of-network provider (which of course increased our deductible and lowered our benefits). Just looking for some insight from those of you wit the same insurance. We have the Kaiser Flexible Choice plan - so we have HMO as well as PPO benefits. Thanks! H.
