What to do, and in what order?

I have already made an appointment with Gagner for a consult in

October...but in what order do I do everything with regards to

paperwork and referrals?

Do I have my PCP refer me to Gagner for the consult so that I can

submit the bill to my insurance?

Does my PCP write a letter of medical necessity along with Gagners

office, and if so, before, after or at the same time?

I'm just trying to figure out what order to do all this stuff in to

maximize my chances of success with BC/BS.
