Hi! I have been reading through the messages and everyone seems so

educated on plagio that I thought I'd ask for some opinions. My

daughter got her scan today and her asymmetry measurement was 7 mm.

They said anything over 5 is considered visible. Does that sound

right? She's only 4 and a half months so the doctor wants me to try

repositioning for 6 weeks and then come back for a rescan to monitor

if any progress was made.

Do you think we have a chance of making significant enough

improvements to avoid helmet therapy? Right now I am basically

keeping her off her head all day long by using a bumbo, exersaucer,

and bjorn carrier. I even hold her when she naps so that she doesn't

put any pressure on the back of her head. Any other suggestions?

Hi-

I think the assessment of 5mm and above being visible would be about

right. We were told by Cranial Tech that 2-4mm of asym is

considered " normal " though I think some orthos say 0-3mm. I think that

with her age and the mildness of asym, you could definitely see some

positive change in 6 weeks of aggressive repositioning. I personally

think it's worth a shot and if you do end up having to get a helmet, 6

months old is still a great age to start, so you wouldn't be losing

any valuable time.

One thing to try besides just keeping her up off of her head, is

trying to add counter pressure to the non-flat side. So if her flat

spot is on her right side, you might want to try (if you haven't

already) getting her to sleep with a sleep positioner on her left side

or at least with her head turned to the left. Or like when you hold

her to nap, have her sleep on her belly on your chest with her head

turned to rest on the non-flat side. If she gets some time to lay on

the non-flat side and add some pressure there it will help the flat

side to pop out, more than just keeping her up off her head all

together.

7mm is pretty mild and could be corrected with aggressive

repositioning. Our specialist (cranio facial plastic surgeon)

recommends banding at 10mm or above, but some band at lower numbers.

It sounds like you are doing well so far, but you also need to keep

her off the flat spot while sleeping. If she's young (below 6 mo) try

a rolled up blanket or sock filled with rice sewed to her jammies to

keep her slightly to one side (so her head falls on the prominent area

not the flat spot). Repo-ing older kids can be done too, but is a lot

harder since they move more. However since the plagio is mild it may

be possible. If the neck is involved (torticollis) this will cause her

to prefer to always look in a certain direction, and you may also need

PT/stretches to make her more comfortable looking in the other direction.
