Guest guest Posted May 16, 2008 Report Share Posted May 16, 2008 I don't think plagio is genetic, although large heads are genetic, and larger heads are more prone to getting plagio (according to our specialist). But it does sound like his plagio and tort started in utero. Medical professionals are horrible at sharing this info. However looking back we did get a paper about plagio from the hospital where my daughter was born - I found it ages after she finished with her band. We adopted her so we had lots and lots of papers coming home from the hospital :-) As for severity, you said your son was 93. That might be his cepahlic index, which is the measure for brachy (flat in back). This is measured by dividing head width by length. Normal is around 80. Plagio measures asymmetry, and 30mm is the worst measurement I've seen. Usually babies above 10 mm should be banded. I don't know about the san antonio locations. Cranial Tech (www.cranialtech.com) has offices in Austin, Dallas and Houston. They do only banding, so their staff is well trained. With other bands such as Hanger or Starband the ortho may do many kinds of orthotics, so experience levels vary. We went to a very good starband ortho in california.Hopefully you'll get some feedback on San . The band is pretty easy to deal with after the first week or so. But, how do they measure plagio? How did they come to the 30mm for plagio? I asked my son's orthotist and he didn't know what I was talking about. I'm praying that's because I didn't know how to communicate my question. Just curious! Which I guess is fine, because it's probably not always great to get stuck on the numbers. But you would think they are at least good as a notable starting point. I think just threw out 30mm as an example. But they measure (with calipers) from the right temple to the back left of the head (there's a specific spot they look for, but I can't remember the official name of it), and then from the left temple to the back right of the head. Then they subtract the lower number (the flat sides) from the higher number (the bossed/prominent sides) and you get the cranial vault asymmetry. My son's was 15mm at the start of treatment and was considered borderline moderate-severe (at CT, other orthos might judge it differently). They can also measure the skull base asym (ear misalignment) by measuring from the right ear to the nose, then from the left ear to the nose, and subtracting those numbers. My son's skull base asym was also 15mm. We also went to a StarBand ortho before banding and she just took measurements of the top of the head and gave me a measurement of 11/16 of an inch, which doesn't equal 15mm, so it seems orthos must measure differently. I think explained how the brachy measurement is obtained. Hope that helps! They are measured on a 30 degree angle - like a big " X " . The measurements should be the same. 0-3mm different is normal. if the difference is 10mm or greater most doctors would recommend banding. Anything above 15 is considered severe. Some doctors also use a percent, but this is less common. For example 100% would mean that both sides are the same. If they two sides were off by 10%, then the symmetry ratio would be 90%. It possible that your doctor is using these numbers, but they aren't used as frequently. If you post some pictures I can tell you what I think. Another concern: I mentioned that the helmet seemed loose. I asked him how the helmet could function as an active device (because I thought it was supposed to be) if it was loose. He said that DOC and STARbands were mostly passive, just there to provide a concave surface for the head to lay on. I told him the D in DOC stood for Dynamic because it was supposed to be using gentle pressure to facilitate progress. I looked on the Orthoamerica website and they called the STARband an active orthosis. Sooooo...isn't that kind of a basic thing? Would you be disturbed if you had had this conversation with the one responsible for your child's banding? Welcome feedback from anybody! Guest guest Posted May 17, 2008 Report Share Posted May 17, 2008 Hi - I wouldn't be as concerned that the ortho referred to the DOC and STAR bands as passive, if he wasn't, in fact, a STARband ortho (if he was a Hanger ortho for instance, you could chalk it up to not knowing much about other brands). That, to me, is a little scary. They are most certainly both active bands. Orthomerica does make 1 or 2 passive bands but the STARBand is not. A passive band is just a plastice shell, and just like he said, a concave shape for the head to grow into. Active bands provide very gentle pressure to hold the prominent areas in place and promote growth in the flat areas, and need adjustments throughout treatment. Where are you going for your STARBand. I know we've chatted before and you're in Eastern NC, but where exactly do you travel for the band. Maybe someone can let you know what they think if they've used the same ortho office before. I thought I put them in an album, but I don't see them there. They are in the new photos space, so if the before picture of 's head helps, you can really see the flat spot. I'm pretty sure the ortho took right front to left back and vice versa measurements. Now all I should have to do is ask him to give them to me and I can do the math myself. That will help me compare what I hear on these boards to what I see in . You can tell from the pictures that he is making progress. That's encouraging! I remember asking him about active/passive once and he was pretty dismissive of it. However my daughter's band (Starband) did fit snugly where her head needed to be held, so in my opinion it was active. Also we were getting good results at each visit. I'm not saying our ortho was right, just that he didn't take that terminology too seriously. Some people put it in their signature file, but then I think it would get sent no matter which group you send too (assuming you are in multiple groups). I looked at 's pics. I would say was probably severe. I don't really know how many mm. You should ask your doc or ortho for the measurements.
