However, keep in mind that every plan is different and it is based upon the agreement with your employer. I was approved (it took about two weeks) and they paid 80/20 after my deductible. The clinic fee was finally paid after a resubmission with additional documentation of medical necessity. I paid about $1000 total out of pocket.
