Nat'l Needs Assessment: Friendly Reminder

***IMPORTANT REMINDER***

Farmworker Health Services, Inc. recently sent Migrant/Community

Health Centers and Migrant and Seasonal Head Starts a mail survey as

part of our National Needs Assessment of Farmworker Health Care

Organizations. As you know, there is little existing farmworker

health data that is either current and/or quantifiable. Your

completed surveys are critically important in our efforts to formally

capture and subsequently report on the health needs of farmworker

families and the needs of health care organizations that serve

them.

If you have not yet had an opportunity to complete this survey, we

would greatly appreciate your participation. If you can complete and

put your survey in the mail by next Friday, May 5th, we will be able

to include your valuable input in the report that we will complete

and distribute in November 2006. If you have further questions
