Dear Jill,

I forgot to mention that I have a four page personal addendum

attached to the " Remarks " section of the application. This

document summarizes my health history from the onset of the

symptoms and problems which led to the diagnosis of acute

pancreatitis, then chronic pancreatitis with pseudocysts, then

brittle diabetes. It includes in chronological order, thorough

descriptions of all my medical procedures, office visits and the

outcome of these visits, and a complete interpretation of the

symptoms and setbacks leading up to my diabetic ketoacidosis

shock and hospitalization. The initial and ongoing symptoms of

both the CP and diabetes are discussed, as are the complex

problems I've had trying to adjust, and live with it all....and how it

has caused my work to come to a standstill. My doctor's letter

is also very comprehensive and he makes it clear that these

diseases and conditions are incurable, with no anticipated

improvement.

Additionally, I have a supportive letter from a very close friend (my

SIL) that documents the " then " and " now " aspects of how these

medical problems have affected me and changed my life. She

was living with us for a few years recovering from traumatic,

disabiling medical problems herself, knew me before CP

entered my life, and has witnessed all the changes and the

negative effect it has had on my life.

I also have already completed all the hospital and doctor's

information and release forms, all the employment history with

job descriptions and dates, personal information and all the

forms for self-employed applicants, etc. Everything's signed and

ready to be mailed.

I have doctor's statements, lab work and documentation

supportive of disability under the SSDI bluebook guidelines for

Section 5 (a) and (B) Section 5.08, and Section 9.08. Yet you still

don't think this will be enough? Then how do other people with

just CP alone get it?

Thanks for the information, especially since it was a bad day for

you health-wise. I know how hard it is sometimes when you're

health's falling apart. I've been hurting more today and last night

than I have in four months, and am so glad now that this is a

phone interview tomorrow...............because I'd never make it if I'd

have to make the hour's drive to the SS office.
