It made me think of a question that was posted recently about using morphine when having SOD spasm, etc. I thought it may interest some of you: (it came from DrugDX I think) B. BILIARY PAIN * 1. Of five studies investigating the effects of morphine on the SPHINCTER OF ODDI, four reported an increase in basal pressure following morphine administration, with one study indicated a possibly dose relationship. However, the one study that did not report an increase in pressure did report an increase in contraction frequency(Isenhower & Mueller, 1998). * 2. Within minutes of intravenous MORPHINE administration, the sphincter of Oddi and the lower common bile duct contract with a resultant increase in intraluminal ductal pressure that may last from 4 to 6 hours. The spasmogenic effect of MORPHINE is readily reversed by NALOXONE (Worobetz et al, 1982). * 3. MEPERIDINE and KETOROLAC do not contract the sphincter of Oddi and would be preferred over MORPHINE for analgesia in patients with biliary pain, or in situations where biliary tract spasm is undesirable (Thune et al, 1990; Krimmer et al, 1992).
