With the evidence documented on paper or a letter signed by the patient's physician, it's very difficult for ER personnel to doubt your words and deny treatment. There is a sample medical records form that Kimber was nice enough to share with us, and it is available for any member to use as a guideline for their own report. This sample form can be found in the files section.
