It has always been the suggestion of the PAI that any person

having an official diagnosis of chronic pancreatitis carry a

medical record sheet with them whenever they have to go to the

ER. With the evidence documented on paper or a letter signed

by the patient's physician, it's very difficult for ER personnel to

doubt your words and deny treatment.

There is a sample medical records form that Kimber was nice

enough to share with us, and it is available for any member to

use as a guideline for their own report. This sample form can be

If you don't have something like this prepared, or a confirmation

letter from your doctor summarizing your diagnosis, I suggest

you get this done as soon as possible. It saves time, opens a

lot of doors, and helps make your complaints credible.
