Cultured staph -- what can we expect?

Hey everyone,

We just got word that our 2 year old son with CF cultured staph and

has an x-ray that shows " streaking, " indicating mucous buildup in the

lungs. He just got over a nasty virus so we're hoping that a lot of

the mucous magically goes away. Anyone care to share what our CF

doctor will have us do now beyond CPT? What you've done to combat

this? How about the issues surrounding staph in the lungs?

As far as I know, staph isn't the worst thing to culture. I imagine oral

antibodics, increased vest. Dr. might be more agressive cause of young age.

I read that everyone has SA - the people that grow a pyseudomoas don't

usually have staph show up (but its there) on their cultures.

Please correct me if I am wrong.

I coined the phrase " BREATHE DAMMIT " ; we have a dog we named -are you ready

for this - TOBI after their medication!!! At least it wasn't albuterol or

Coly...

Matt,

My 3 year old occasionally cultures staph., then the next time it will be

gone. My clinic does not consider it a big deal. Once they gave him

antibiotics because he had a little runny nose with it. Otherwise if he

doesn't have any symptoms they don't do much of anything. Staph is

something that even those without cf get, not just a cf thing. The

" streaking " in the lungs I don't know about. Like you said, hopefully it is

leftover from the virus. They all will eventually start showing some signs

of cf in the lungs however.

As Angie said, staph aureus, often is not treated unless it appears to

be causing problems. There is a difference among

1. culturing staph (in the sputum but may or may not have established a

colony in the lungs)

2. colonizing staph

3. being infected with staph (causing inflammation/an exacerbation)

Staph can come and go because even if it is eradicated, it is so common

in the population that it is easy to pick up again OR the person with CF

may have it growing on their skin (armpits, groin, etc.) and easily

recontaminate their lungs unless antibacterials are used to also

eradicate it from the skin.

Over time staph a. is a pathogen that does do damage to the lungs but it

is debateable how serious that damage will be in individual cases.

IF staph causes exacerbations, it becomes necessary to treat it with

antibiotics (IV or oral). In many cases, chronic use of oral antibiotics

(such as cloxacillin, flucloxacillin, keflex) are required to keep staph

aureus under control..

Staph aureus is one of the three most common bacteria cultured by people

with CF (hemophilus influenza and pseudomonas a. are the other two).

The CFRI conference notes from 2000 (someone recently referenced them on

this list or another list - Kim Payne?) indicated that Dr.Strandvik from

Sweden treats staph a. aggressively, considering it often to be as

damaging as pseudomonas. In North America, it does not seem to be

regarded as seriously by most unless it is causing infection. In fact,

we have found some doctors tend to think that a colony of staph a. can

be helpful in keeping pseudomonas at bay or from overgrowth..

Staph a. can develop a resistance to antibiotics (developing into MRSA)

but this is usually over a very long term (decades).

They often do prescribe oral antibiotics for staphylococcus, but my doc

tor has always said that it was not the BIGGIE bug, and not to fuss too

much about it; she relies on ct or EBCT scans to determine bronchiec

tasis or permanent fibrotic scarring.

Many ordinary oral antibiotics work well on staph. My daughter is having

good luck with Augmentin. If he's not on antibiotics I imagine he should be.

Can't think of any other advise!
