I am very tired tonight so I will be very brief. I still am being told by my

DR. That the Puestrow is the way to go. Yet he did tell me that the only one

that should do it is Dr. Lehman or Dr. Scherman. Dr. Lehman is who I wanted to

see months ago, but it was for duct work. I am told that he and his group are

having great successes with the Puestrow. My question to the group is, has

anyone had Dr. Lehman or his group d the Puestrow on them and was it successful.

According to litature, they are making great strides with this procedure. I

know that Dr. Lehman is excellent in stent work, but is he just as good in doing

the Puestrow? I would appreciate any feed back if anyone has anything to

share. I am also having a second opinion at Hopkins with Dr. Cameron who is

known

for his ability with the whipple. Do you stretch your ducts and keep the organ

or do you slice and dice the organ and lose other organs? I am at quite a

crossroad............Paget

The longitudinal pancreaticojejunostomy or Puestow's procedure is the

prototypic drainage procedure for patients with marked dilation of the main

pancreatic duct (greater than 7-8 mm). An 8-10-cm segment of the pancreatic

duct is unroofed and intraductal concretions removed (Figure 22A). The

jejunum is divided (Figure 22B) and the opened pancreatic duct is

anastomosed to the jejunum (Figure 22C). This allows adequate drainage to

enter the jejunum. A jejunojejunostomy reconnects the jejunum to restore

continuity of the gastrointestinal tract (Figure 22D). This procedure is

successful in relieving pain in 70-80% of patients in the short term.

Pancreatic function remains good because there has not been resection of the

gland. It is a safe and effective surgery with low morbidity and mortality.
