Hello, this is Dean Walters again. I'm sorry about not responding but

I have been very busy with my wife, , and work and well you

know. I am writing because is back in the hospital again. I

received an e-mail about a doctor at Northwestern Hospital in

Chicago. I am trying to get that info again as my computer crashed

and I lost everything. What else can go wrong (LOL). Hope to hear

from someone and thank you for your help.

There was next nothing in the e-mail to know WHO the doctor is that

this person is trying to get info on. So, here is the best I could
