It could be nothing or it could be a secondary infection. At the very least, often antibiotics don't work the first time, especially with ear infections and they need another dose. Congestion in nose can result in ear infections too so even if he's been cleared up from the first one a new one could be brewing. I wouldn't run like the wind immediately to the doctor's in a state of panic () but I would go back and have it checked. By the way, you are not being paranoid at all.
