At the last visit when so many things were being decided, I was not there, our other child was sick so, my husband, Vince, took her. I will ask him about a yellow sheet. I tried to call and email Dr. H., but on 3 occasions have not gotten a live voice. I appreciate your help. Happy Thanksgiving!
