Guest guest Posted October 16, 2003 Report Share Posted October 16, 2003 on the way out of the hospital, i passed by Admitting & picked up their " Advanced Directive " a.k.a. living will. it was a 1 page form, where you name you health care proxy or surrogate, & let them know if you want to be resuscitated in case of incapacitation. why did i bother? i'm having surg in the upcoming months. lori h. 17 1/2 months out Quote Link to comment Share on other sites More sharing options...
