I'm guessing that you as a rep would be informed about all these...eh? One thing that I tried to get a handle on to aid in my appeal process was total numbers of resurfacings outside the US and numbers inside the US. MFRs. for some reason hold that information close to the vest. Despite being able to come up with " hard " numbers, I believe they are sufficient to support the validity of resurfacing. Most of the resurfacing devices have had more procedures performed than the most new creamics that have been approved only since mid-2003. Ask hubby why they are approved by the FDA and resurfacing devices are not. What I found most reassuring in my decision to choose resurfacing was the information I found on THR failures and dislocations (especially for younger patients). For further reinforcement you need to consider that the majority of THRs are performed on people on the average much older than us that are not active. When comparing those failure rates to the comparitive lack of failure/dislocation rates for resurfacing patients (typically much younger and much more active) IT'S A NO BRAINER!!!! Seriously, good luck with your decision and let me know if you need any help with the links. The next one I have done will probably have to be done in Vancouver Canada (cost prohibitive for going back to Belgium....). Can you tell me which orthopaedic surgeons in Vancouver use the Birmingham? I have heard recently that some of them are switching to one called the Durom, don't know why, maybe it's cheaper? It's like the old saying about statistics " Figures lie and liars figure. " That said I still believe that what's out there to support resurfacing. My insurance company in their denial of my claim, consulted with an " independent " third party whom said that the traditional THR is the " gold standard " for hip surgeries. The only device that has the numbers to support a " gold standard " endorsement is the small metal ball with the poly cup and we all know the limitations (dislocation and loosening) of that for the active 65 and younger crowd. The only THR that is worth looking at is the large MOM THR and I don't think that is approved by the FDA either (although COC may have more numbers (and approved by the FDA) in the US and a greater OS support group). That said, it's still a THR making your next THR a secondary instead of a primary THR if you had resurfacing as your first replacement. My doctor is Dr. Garbus and two of the others are: Dr. Grreidanas who only does resurfacings, and Dr. Clive Duncan. I beleive that they are a team who share the same office space and operate out of the Vancouver General Hospital. I am scheduled for the Durom in 18 months because of the long waiting list. The Durom is approx $1,000 Canadian less than the Birmingham but it is not the lower cost for the reason that they are using it, I don't think. A friend just got one from Dr. Barry Waldman at Sinai with a MIS technique. She is very happy with it. I think there are several other manufacturers with M-o-M THR devices that are FDA approved. Dr. Gross mentioned to me that Corin has one but it doesn't go up to the larger sizes (of femoral heads) that the Cormet 2000 resurf does - I think he said 38mm was the largest head. That, too, is an FDA issue - apparently they have not gotten the larger devices approved yet. I wonder what numbers they were able to put together for trials for the BFH as opposed to the resurfacing device. Maybe someone knows the details of the FDA approval process for the BFH. Seems that approval for resurfacing would be a no brainer if the BFH is approved, as the femoral component of the resurfacing device is approved as a hemi and the acetabular components are the same for both devices. Perhaps I'm missing something else? I didn't ask who the manufacturer was and didn't care. My only concern was that if it had to be a THR that it be the large M-o-M THR instead of any other option. Could also be that the smaller Corin M-o-M THR device that is referring to is approved by the FDA but that the larger Corin M-o-M THR device is not approved. I guess if you're comparing a non approved resurfacing device to a non approved THR device ... maybe it doesn't make any difference to the insurance companies. Here are 6 medical websites with bona fide medical info that she sent me. When I was researching resurfacing about a year ago, I had no problem pulling up research studies-some comparative with THR. However, it is worth noting that analyzing medical research is a skilled activity. I am not a doctor, but work in a hospital and regularly attend our research meetings. Medical studies are taken and then analyzed with a series of variables (such as size of sample, control groups or not, design of study, etc. etc.) to see how reliable/accurate/ predictive/useful they are.. (Doctors using this site might want to comment.) Having said that, I seem to remember that one of the British papers does just that---compares various research studies on resurfacing and comments about their usefulness. I wonder what he would do if it was his hip. One thing that I tried to get a handle on to aid in my appeal process was total numbers of resurfacings outside the US and numbers inside the US. MFRs. for some reason hold that information close to the vest. Despite being able to come up with " hard " numbers, I believe they are sufficient to support the validity of resurfacing. Most of the resurfacing devices have had more procedures performed than the most new creamics that have been approved only since mid-2003. Ask hubby why they are approved by the FDA and resurfacing devices are not. What I found most reassuring in my decision to choose resurfacing was the information I found on THR failures and dislocations (especially for younger patients). For further reinforcement you need to consider that the majority of THRs are performed on people on the average much older than us that are not active. When comparing those failure rates to the comparitive lack of failure/dislocation rates for resurfacing patients (typically much younger and much more active) IT'S A NO BRAINER!!!! Seriously, good luck with your decision and let me know if you need any help with the links. I know from my surgeon that these don't tell the story that is happening...........partly because Resurfacing is catching on so quickly figures are out of date re trends before they are published............ I know 2 years ago my brotherinlaw aneastheastist hadn't heard of a Resurface but now regularly attends operations of them as the aneastheastist........... and partly because of so many people are unsuitable for a Resurface either because their femur bones wouldn't carry it or they are too old........... very few cases would be because they preferred to have a THR instead of a Resurface.............and I suspect even that is fast changing as OS are giving in to the demands from their patients and going off for training..........so there are less and less enthusiastic THR OS setting out to convince their patients to have a THR instead of a Resurface..........the major reason people here would now settle for a THR instead of a Resurface given the choice........... My surgeon just says that in due course it will be......... can the bone take a Resurface, if so that will be done, if not then whatever choice of THR is next suitable............ Wonder how many years it will take the US surgeons to reach that point...........?? Then he uttered something about being in the most regulated industry in the world. The whole reason this came up was that prior to running into an insurance problem, I was planning to go to Dr. Mont and he and I talked about using the BFH if he was unable to do a resurf. When I went to Dr. Gross I mentioned that was my original plan and he warned me that if he needed to do a THR that he did not have access to a device with a femoral head as large as the BFH. He went on to explain that if I ever needed revision he could then use a large ball femoral device and he indicated it was some problem related to FDA approvals. I am 52... so i'm sure you are in the ballpark at 54. I had to have a bone density test to make sure that the bones were strong enough. The bad news in Vancouver is that the waiting lists seem to be long ( 6 months minimum for a consultation, then a 9-12 month waiting list for the operation). It is covered by Pacific Blue Cross, but I have to pay $3000 extra for the hardware(?!) The good news is that there is a great pre- operative patient education program with lots of different classes on everything from managing your pain/ fatigue , stress management, communication skills, dealing with emotions etc etc and a pre-op. physical therapy program. hope this helps. from Irene in Vancouver. Depends on how active you want to remain and for me giving up downhill skiing for the rest of my life was unacceptable - as well as the other restrictions THR puts on a person.
