In addition to what said, you ALSO need to include you pre-approval letter from the insurance company. You can't get a date without one, unless you are self pay. I talked to Debbie recently and she and Barbara are doing the approving of packages in Dr R's absence. She said that she is probably a little more strict on the family support letter. PLEASE make sure their letter addresses their understanding of the risks of complications, including death. I had Blue Cross, Blue Shield of AL. Who are you insured by? I'm in 'Bama, too. The patient information form that you email is used to get insurance approval. In the meantime, you can be getting your " packet " together. This is: (1) Your patient letter - this is the biggee -- it has to be 5-10 pages long, showing you understand the risks of obesity and the risks of surgery, as well as the requirements to have the surgery and what you must do the rest of your life after surgery. It is covered in more detail in the patient manual. (2) Physical (including a pap test, mammogram, and EKG within the last year. (Not records of colds and flu, but treatments for things like arthritis, any surgery, hormone disorders, depression, heart disorders, gallbladder, ulcers, steroid treatment, cancer, anemia, stomach disorders, big things and actual disorders of your body) (4) Primary Care Physician ( " PCP " ) support letter. (5) Family Support Letter - A letter from a family member, also showing the family member understands the risks of obesity and the risks of surgery, the long term requirements after surgery, and is supporting you in this decision. (6) Patient contacts - information showing you have had contact with other patients prior to surgery (7) Two pictures of yourself with clothes on, one sideways, the other full front, both of them full-length All of this is sent in at the same time to Dr. Rutledge's office. I may have to be self pay since my insurance is being really nasty. I believe you should go ahead and get your packet together and get it to Dr. Rutledge since he needs a few days to approve it also. I just talked to Joanne, who is a travel agent in Tennesse who had the MGB on June 9. She said she thought the neighborhood around the Duke Towers was questionable, so I am not staying there. She stayed at the Fairfield Inn for $39 a night, and she said that it is only a few minutes away from the hospital, and there were a lot of stores and restaurants right in the area. That is where I am planning to stay on July 10. I just found out today that my insurance won't approve me until I have seen Dr. R, so I am going to Clinic on July 11.
