One parent used a leather punch and punched holes in a circle and then cut out (like perforations). I never actually did it myself. The orthotist did. They have lots of groovy leather tools in the brace shops ya know. Good luck!! We are in Australia and the orthodics dept at the childrens hospital automatically do it - I thought it was a normal to have a hole where the heel is.
