Darla,

Grace had similar problems when she was younger. For her, she was unable to tolerate my breastmilk. She also had severe reflux and would often vomit. She always had severe diarrhea, having very watery BM's at least 8-10 times a day. Do you ever notice any puss in her stools? Grace often had a lot of puss because her stomach was torn up, because she could not handle anything. Her GI put her on a formula called Neocate. It is non milk no soy and is amino acid based. It is g tube friendly also. For her it was a huge help. I guess it is commonly used for diarrhea and will actually thicken the stools up a bit. My G.I. told me that it is supposed to be the best, and if it did not work nothing would. I would defiantly consult her GI about the problem. Also bring up the Neocate, if they don't offer it. It is not prescription but it is best to get it through a medical supplier. It has warnings on it that it should only be used under the care of a doctor. Be forewarned it is extremely expensive, the can is about $42 and you use twice as much as you do with normal formula. (We went through a can every other day.) Also to keep her hydrated we gave her Pedialyte all day, every day. We gave her the Pedialyte like you would give a baby juice. I can't remember exactly but I believe we gave her about 3 or 4 ounces between feeds. She was unable to eat any form of solids until she was almost 9 months so she was completely on a liquid diet of Pedialyte and Neocate. The first time that Evan drank milk based formula (he drank 5 ounces),

he had stringy mucus filled diarrhea. I nursed him exclusively; but

because I had no reason to suspect a potential problem with formula, I

tried formula when my mother-in-law watched Evan when my husband and I

went out to dinner. Evan was also allergic to soy and nutramigen (corn

based). When he quit nursing he drank Alimentum until he was 3. It

was expensive, so his allergy doctor wrote a script for it and our

insurance paid 80% of the cost. I believe that the supplemental

carnitine in the formula kept him from developing symptoms because he

showed some minor decline after he stopped drinking formula but before

his big loss of skills when he turned 4. It is probably a good idea to

stop the formula and use pedialyte instead. Porrah can nurse also.

It is such a blessing that you can tube feed her now.

Zipporrah is really having liquidy diarrhea and

I am worried if it goes

on very long that she will become dehydrated. She is also vomiting

every time she has a feeding. There is a lot of phlegm amonst the milk

as well. At first her bowels were light yellow with large "curd-like

spots." Then it turned more greenish and thinner and now it is pure

liquid that is clear looking.

Has her stool been cultured? It could be rotovirus or staph. Staph is

a bacterial infection, so if could be treated with antibiotics. Once

Colin got staph (he was 4) and he had to wear pull-ups because he kept

having diarrhea while he was vomitting. Poor kid was truly

offended! There have been some studies with positive results using

probiotics in infants. Inflammation from allergies can impair

mitochondrial function. One of the docs at the UMDF conf in Dallas

mentioned tumor necrosis factor-alpha as being one type of inflammation

that can cause problems. It decreased significantly in the infants who

were given probiotics from

709 pg/gm to 34 pg/gm.

BACKGROUND: The gastrointestinal microflora is an important constituent

of the gut mucosal defense barrier. We have previously shown that a

human intestinal floral strain, Lactobacillus GG (ATCC 53103), promotes

local antigen-specific immune responses (particularly in the IgA

class), prevents permeability defects, and confers controlled antigen

absorption.

OBJECTIVE: The aim of this study was to evaluate the clinical and

immunologic effects of cow's milk elimination without (n = 14) and with

(n = 13) the addition

of Lactobacillus GG (5 x 10(8) colony-forming units/gm formula) in an

extensively hydrolyzed whey formula in infants with atopic eczema and

cow's milk allergy. The second part of the study involved 10 breast-fed

infants who had atopic eczema and cow's milk allergy. In this group

Lactobacillus GG was given to nursing mothers. METHODS: The severity of

atopic eczema was assessed by clinical scoring. The concentrations of

fecal alpha 1- antitrypsin, tumor necrosis factor-alpha, and eosinophil

cationic protein were determined as markers of intestinal inflammation

before and after dietary

intervention.

RESULTS: The clinical score of atopic dermatitis improved significantly

during the 1-month study period in infants treated with the extensively

hydrolyzed whey formula fortified with Lactobacillus GG. The

concentration of alpha 1-antitrypsin decreased significantly in this

group (p = 0.03) but not in the group receiving the whey formula

without Lactobacillus GG (p = 0.68). In parallel, the median (lower

quartile to upper quartile) concentration of fecal tumor necrosis

factor-alpha decreased significantly in this group, from 709 pg/gm (91

to 1131 pg/gm) to 34 pg/gm (19 to 103 pg/gm) (p = 0.003), but not in

those receiving the extensively hydrolyzed whey formula only (p =0.38).

The concentration of fecal eosinophil cationic protein remained

unaltered during therapy.

CONCLUSION: These results suggest that probiotic bacteria may

promote endogenous barrier mechanisms in patients with atopic

dermatitis and food allergy, and by alleviating intestinal

inflammation, may act as a useful tool in the treatment of food allergy.

Evan has been taking Lactobaccillus GG for several years. It is sold

under the brand name Culturelle.

BACKGROUND: Reversal of the progressive increase in frequency of atopic

disease would be an important breakthrough for health care and wellbeing

in western societies. In the hygiene hypothesis this increase is

attributed to reduced microbial exposure in early life. Probiotics are

cultures of potentially beneficial bacteria of the healthy gut

microflora. We assessed the effect on atopic disease of Lactobacillus

GG (which is safe at an early age and effective in treatment of

allergic inflammation and food allergy).

METHODS: In a double-blind, randomised placebo-controlled trial we gave

Lactobacillus GG prenatally to mothers who had at least one

first-degree relative (or partner) with atopic eczema, allergic

rhinitis, or asthma, and postnatally for 6 months to their infants.

Chronic recurring atopic eczema, which is the main sign of atopic

disease in the first years of life, was the primary endpoint.

FINDINGS: Atopic eczema was diagnosed in 46 of 132 (35%) children aged

2 years. Asthma was diagnosed in six of these children and allergic

rhinitis in one. The frequency of atopic eczema in the probiotic group

was half that of the placebo group (15/64 [23%] vs 31/68 [46%];

relative risk 0.51 [95% CI 0.32-0.84]). The number needed to treat was

4.5 (95% CI 2.6-15.6).

INTERPRETATIONS: Lactobacillus GG was effective in prevention of early

atopic disease in children at high risk. Thus, gut microflora might be

a hitherto unexplored source of natural immunomodulators and

probiotics, for prevention of atopic disease.

We also use Neocate Jr. and Plus One for our son Caeden. He is getting most

of it by G-tube. It is packed with calories 30/oz and has lots of vitamins.

Agreeable it is very expensive and costs us nearly $1000 / month. We however

can't get it here in NY without a prescription. We order it through our

pharmacy. The company who dispensis it SHS also has other GTube soultions.

Caeden is very allergic to milk and this was the only formula he could have.

As everyone has said, I would definately removed the formula. At

least for a couple of days and then you can see if the runny bms and

vomiting stops. My daughter Molly has food sensitivities and was on

soy milk. She still never had a normal bm (4-5 day and very green

and runny). Finally her GI found these sens. and took her off of all

of these things (wheat, milk, egg and peanut and minutely soy). Her

bm's are now normal after 2yrs. We are trying to get neocate covered

through medicaid to put in her soy and rice milk. It gives added

protein and is good for kids with protein sensitivities.Great amino

acids!

Pedialyte is good and I would try a few oz every hour. Slow push or

drip. When is sick, if we give it to her very very slow she

can keep it down.

If you remove the formula she is still having diarrhea, then it

could be the rotovirus. had this for a good 10 days when she

was 9mos. She still had an appetite, but whenever she ate, she threw

up. Very green diarrhea that lasted forever. This was before our

mito diagnoses and they would not admit her for dehydration.

Ihope she gets better and is able to keep some fluid down. If she

cannot, call Whiteman and have him try to get her direct admitted.

If he talks with someone at your hospital, he should be able to do

this.

This sounds similar to my experience w/ rotovirus. My oldest daughter (she was 18mos. at the time) and I had it at the same time (I was 6 mos. pregnant w/ Maddie) and my youngest daughter had it last year. It went along with the colors you described, except it had the most foul, rotten egg like smell! (As well as severe stomach cramping.) I think I remember the diarrhea lasting about 10 days, vomiting lasted 1 or 2 days. HTH. I think of your family daily and truly admire your strength and your faith.

Zipporrah is really having liquidy diarrhea and I am worried if it goes on very long that she will become dehydrated. She is also vomiting every time she has a feeding. She has been getting much more milk now than before so at first I thought her output was due to that and then she became irritable after feedings more and more. She seems to be having gassiness and pain during her feeds and I am unsure how to help. I don't know if this is part of her illness since she has a fever, or if it could be the formula, or a combination of both. She is pretty crabby and things are only getting worse. Any help would be appreciated. ER probably wouldn't understand! I want to keep her home as long as it is safe, but don't want to let her get too dehydrated. I am giving her 5 oz. every 3 hours except ot night she goes 6-8 hrs. without feeding. (Other than an occasional nursing snack.) Is this enough to keep her hydrated? I may add pedialite during her sleep with a pump but am afraid she will only loose it again. Lucas has been tube fed for almost 4 years. Lucas has problems with intolerance of certain formulas. I would recommend a slow progression to full strength of the supplement you are giving. Also if she does indeed have a virus milk based products can wreck the most havoc on the GI system. Maybe you can increase one ounce at a time of formula to pedialyte and increase the amount every 2-3 days. Being exclusively on pedialyte for too much time is not necessarily the best either.

Sounds like it could be rotovirus. The stools can be really watery.

The only way for you to know is to take a diaper to the lab and have them

test it. Leah had this twice and needed to be hospitalized both times

with an IV. So I would be concerned for Porrah. Get it checked

out. Can't remember if there is a weird smell in the stools.

But remember the real watery stuff.

Hi Darla:

Sounds like it could be rotovirus. The stools can be really watery.

The only way for you to know is to take a diaper to the lab and have them

test it. Leah had this twice and needed to be hospitalized both times

with an IV. So I would be concerned for Porrah. Get it checked

out. Can't remember if there is a weird smell in the stools.

But remember the real watery stuff.

Is this enough to keep her hydrated? I may add pedialite during her

sleep with a pump but am afraid she will only loose it again. Any

suggestions?

Darla, What kind of formula is she on? Does it have fiber in it? When Chelsea was first put on Pediasure (at 3 yrs), it was without fiber and it gave her liquid bm's. When she was switched to Pedaisure with fiber, her bm's became solid, too solid, actually. It usually turned into a rock in her inrestines!
