Hi there:)

Yes. I did recieve a bill for about 1500 dollars for pre-surgical

work up. (x-rays,molds,splint,consultation etc..) Though I sent it to

my insurance company, and they are paying about 80% of that:) You can

try and send it to your insurance company, and see what they do with

it. I am Canadian, so that might make a bit of differance.
