Guest guest Posted December 29, 2003 Report Share Posted December 29, 2003 Not sure about Singapore's surgery stats, but usually with this surgery, (orthognathic) you need to wear braces 6-9 months in advance and then at least 6 months after. The surgery itself is about 5-6 hours at the most I believe, with a hospital stay overnight..at least.. Then you are banded shut/wired shut or splinted shut or a combination, for 5-6 more weeks after that. It's not a quick procedure, but if you go through with it you can expect to see the smile you want in a year. However, that last 6 months you are in braces your jaw is already corrected, so you should definitely see a difference there. Guest guest Posted December 29, 2003 Report Share Posted December 29, 2003 My surgeon and orthodontist both said they tell all their surgical patients they could be wearing braces for 1 to 3 years both before and after surgery, for a max of 6 years total. That's probably a very conservative view, and many do not have to wear them for that long (including myself - 17 months before surgery/about 8 months post- op), but I'd rather be prepared for the worst, personally. As for cost, the surgery alone was around 16k USD. Braces were another 5k. My insurance covered nearly all the surgery, but not a penny of the braces. I won't tell you not to have the surgery for cosmetic reasons, but I will caution you that the procedure has functional consequences. If you have functional problems with your bite now, it might help, but if you do not have funtional problems, you run the risk of developing them as a result of your repositioned bone structure. If you have healthy functioning jaw joints right now, just be aware of that risk. Malfuntioning jaw joints are very painful and very hard to fix. I know this from experience, though I had joint problems long before my orthognathic surgery. The whole process is very challening, both emotionally and physically. I recommend that anyone thinking about this surgery try to gather as much information as necessary before beginning the process in order to make an informed decision about whether or not it is right for you. 'Tisn't that everybody who had pain before surgery will get it solved. 'Tisn't that everybody who had no pain before surgery will have surgery and avoid pain later. It's that none of this is a perfect science. You look at the odds. You ask the questions about risks. And then you figure out what you think is worth doing, with the advice of your specialists. And if the cosmetic changes are important to you, factor them in. If they are not, ignore them. It (jaw trouble) can get better, or it can get worse. Your bite will probably get a lot better. Your profile will probably get better. But I don't think anybody can predict the rest of it. So get the best opinions you can find, and then make up your own mind about what you want/need to do. gives good and measured advice, I think. There are no guarantees for any of these procedures, except that the docs are experienced and expert, and should be expected to do the best jobs they can for you. Best wishes. C. Recently, I went to see Orth to correct my problem but to my surprise, it's not so simple for my case. Orth said I've short chin and even if I've my teeth corrected by braces, the result will not be satisfactory. Therefore, he suggested me to see an OS. I've seen 2 OS which recommended by him. Both OS have different ways/methods to have my jaws corrected. 1st OS (National Dental Centre - Dr Teh) Her methods was, she will move both my jaws. My upper jaw backward and have a lower jaw advancement. This will cost about S$ 3,000. Because I've very thin jaw bones, she said the chances I will have a relaspe is very high. And it will defeat the purpose of doing it. So she will do a detraction on both of my lower jaw to make the jaw bones grow. (Each detraction cost approx. S$1,500 and I need 2 of these thing). For this method operation time is 5 hrs and I've to wear the " thing " for 3 months before taking it out. Followed by braces for another 6 months. 2nd OS (Mount E - Dr Winston Tan). His way to correct my jaws is to move back my upper jaw and have a lower jaw advancement. I will have total 8 Titanium plates on my jaws/mouth. He will bind my teeth for 2 weeks and will take it off in 6 weeks time. He will also over- correct my jaw for prevention of relaspe in future. But he told me even if I were to have a relaspe, the percentage is about 15% different of my corrected jaws. - Eg. if he move 1cm of my lower jaw forward, the most it will move back is approx. 1.5 mm. The cost of this is approx. S$ 13,000 staying at hospital for 2days 3 night. Operation time is approx. 3 hrs. For braces charges, mine is about S$4,000 for metal and S$4,700 for ceremic. But he told me even if I were to have a relaspe, the percentage is about 15% different of my corrected jaws. - Eg. if he move 1cm of my lower jaw forward, the most it will move back is approx. 1.5 mm. The cost of this is approx. S$ 13,000 staying at hospital for 2days 3 night. Operation time is approx. 3 hrs. For braces charges, mine is about S$4,000 for metal and S$4,700 for ceremic. Need ot wear braces for 12-18 months, followed by jaw surgery and lastly wear braces for another 6 months and I will have a perfect set of teeth. FYI, I'm still considering. But this sat, i will see them again. Cos' I've got a few more questions to ask him after reading and know more on this jaw surgery. Disadvantage of this surgery is, lower chin might have temporary or permanent numbness after the surgery. The rest he didn't say much. But I would really like to know what other problems I will face beside this. Any adviser?? Have you seen any orth or OS?? If so, could you tell me your cost and the method they use on you. P.S : Others who have their jaw corrected, will there be any side effect, discomfort or pain after it's completely heal? Can we still laugh, eat and talk normally like before?? It's very possible that you would not have any pain after surgery, but no one can guarantee what your results would be - not even a surgeon could do that. As Cammie says - this is not an exact science. I, personally, would not have had the surgery for cosmetic reasons. For me, all of the months in braces, the time in the hospital, the long recovery and the financial cost would not have been worth cosmetic benefits to me. I have had some cosmetic benefits as a result of the surgery, but those were never my goal. I had horrible muscle tension all over the left side of my head before my operation. The muscles were trying to protect my arthritic left jaw joint, but now that tension is much better, but my joint pain is much worse. Still, I'd do it again if I had to. I'll probably have to have open joint surgery at some point in the future to fix or remove the cartilage disc in my left joint, but I'm hoping that my improved bite will help make that procedure more successful when it comes to that. If you haven't already had a consultation with a surgeon, I highly recommend that you do that. An oral surgeon will be able to answer questions directly as they relate to you. Those of us at OSS1 can only tell you what our personal experiences are, but a surgeon will be much more equiped to tell you what you could expect from the surgery. For my daughter this is the course of treatment that was followed: 1) One year + before hoped for surgery - all four wisdom teeth were removed. Some OS take them out at the same time as the surgery. Ours wanted the bone healed for a year before jaw surgery. 2) Xrays taken of wrist growth plate and jaw length. These have special names, ask your ortho or OS. By these xrays they can determine about how far along you are before your growth plates close and no more growth is expected. 3) Braces on for 8 months before surgery. Everyone is different. 4) Xrays taken again 4 months after first set to see growth that has taken place. 5) Xrays taken again 4 months after first set to see if any growth has taken place. Our OS would not perform the surgery unless no growth had taken place in the jaw area in the last 4-6 months and all the growth plates were closed in the wrist. 6) Surgery. 7) At least 6 weeks recuperation before bones are healed. 8) Braces until teeth and bite are were they need to be. 9) Braces off and hopefully no more thinking about the bite, etc. We say hopefully because there are no guarantees. No surgeon should/will do this surgery until you have stopped growing.... we have had the same set of x-rays etc as Juli's child. yesterday she had 2 wisdom teeth removed, gives me a glimpse of what my summer will be like !!!. but she is not having her surgery for cosmetic reasons... she will have both upper and lower jaws done and her chin fixed. apparently after all the films were done the doc said that her smile was also crooked ( not to my eye) and that as long as they were fixing the bottom jaw we should fix the top and then the chin has to be moved . so far the chin is not covered under my insurance but he wil rite letters hoping to change their mind. the rest of the bills are covered under our insurance, we have bCBS. hoping all is well with everyone. She had braces initially for four years then after 1.5 years with braces off, she had moved significantly so that none of her teeth touched - no biting surfaces, hence, no mastication was taking place so digestion was the pits. She was Utah's Junior National Teenager for 2002-2003 so this definitely was not cosmetically done, we just hope she will be reconciled to her 'new' looks when all the swelling is gone! Her attitude is good so this should not be a problem - X (fingers crossed!). Our insurance would not pay anything! Ugh! Fortunately, our OS and ortho were very reasonable and we had room in our home loan to refinance! So happy for these youngins that doctors know to check for growth before this horrendous surgery so the chance of having to do it again is minimal! Keep us informed how your daughter does. I'll probably have to have open joint surgery at some point in the future to fix or remove the cartilage disc in my left joint, but I'm hoping that my improved bite will help make that procedure more successful when it comes to that. If you haven't already had a consultation with a surgeon, I highly recommend that you do that. An oral surgeon will be able to answer questions directly as they relate to you. Those of us at OSS1 can only tell you what our personal experiences are, but a surgeon will be much more equiped to tell you what you could expect from the surgery. The statement you do not understand means - do not have the surgery at all until your doctor can see through xrays, measuring growth, etc., that you have a good chance not to grow any more. Most men have finished growing by age 22, most women have finished growing by age 16. By taking xrays of your wrist and your jaw, and then taking the same xrays again in 4 to 6 months, the surgeon or orthodontist can take measurements to see if you are still growing. On your wrist growth plate xray, the doctor can actually see if your growth plate is filled in or not and then tell you how much growth you have left to do. The doctor can actually graph when you will be finished growing by taking these xrays at intervals. None of this is completely accurate because there are some that have a growth spurt later on in life - this is not normal though, so usually by taking these xrays and 4-6 month intervals they can know pretty sure if you have stopped growing or not. I only said that I would not have had the surgery for cosmetic reasons alone, especially now that I've been through it and know what it's like. Please do not put words into my mouth. I do not pass judgment on others whose cosmetic concerns outweigh functional ones, but regardless of what your concerns are, you are at a point where you would be best served to have a consultation with a medical professional who can speak with you about the specifics of your own case. The growth issue is not about whether or not jaw bones will stop growing. It's about the person undergoing the surgery to have stopped growing before the surgery is performed. Think about it - if you're still growing (as in the physical changes we all go through as we age into adults), then your jaw bones are still growing too. If your bones are still growing, then the doctor does't know what your final size and shape will be from nature, so how can he/she preform the best surgical repositioning of bones that will still change via natural growth on their own after surgery? If you can't get along with ANY of them, they may not be the problem, and you need to think some more. But I'll bet you can find someone you can work with pretty easily and successfully, if you can manage the financial and emotional aspects of all this.
