Guest guest
Folks, Washington State Department of Health is proposing to make changes to it's vital statistics certificates. These affect what information will be collected and the form of that data. For anyone using these forms or those of you who use vital statistics information from other states and might want some comparative data -- the WA Department of Health is soliciting comments on what's proposed. See below from their website: " Due to recent changes in federal standards, the Washington State Department of Health is required to revise three vital statistics certificates: Birth Certificate, Death Certificate, and Fetal Death Certificate. The purpose of this website is to solicit public comment on proposed changes. We welcome your feedback! Please see Instructions for the Certificate Revision Process. Deadline for comments is June 29, 2001. " Website: http://www.doh.wa.gov/EHSPHL/CHS/CHS-DATA/certrev/certrev.htm Alice C. Larson, Ph.D. las@... 206-463-9000 (voice) 206-463-9400 (fax) P.O.
