He needs the following to be tested on a regular basis - TSH, free T3 and freeT4. Once he's stable it may not be necessary to do this more than twice a year. If he had been hyper but was now euthyroid for an extended period of time then an endo might be justified in testing only the TSH for screening purposes. But, since a hypo patient's hormone replacement is a lifelong proposition all levels must be tested when testing is to be done.
