The device knows (I believe it measures muscle density or fascia density or who knows what) and has a special beep to alert you - then you just push the button and you get a small shock. sometimes it hurts, sometimes it doesn't. If it hurts, just pull it away (you might do this automatically) and you can pulse it, put it on, take it off, put it on, etc. You apply no pressure because the electrical shock does it for you. I bought mine from bodyclock http://www.bodyclock.net/Acupuncture/ It comes with a small pamphlet like book that explains it. If you go to that site you will find a much better description than I can give. I actually have the stimplus pro - not cheap - but well worth it in my opinion. It isn't necessary but it is useful if you want to keep the beeping sound private. I rarely use it. The shock is sorta like a TENS unit, but sorta not, too. (how's that for helpful). It is very point specific - not spread out like the TENS - and there is also not a pulse feel to it. If you hit a particularly tender area it can be way more painful and then again, sometimes you can't even feel it. Also, sometimes I can feel the muscle contract very strongly and then completely release - I don't get that with the TENS unit.
